We would like to extend a heartfelt congratulations to all of our students at LACHSA. The 2009-2010 school year was eventful and filled with lovely concerts, gallery displays, recitals, concerts, academic and arts award ceremonies and of course our final culminating event: graduation at Disney Hall.
Although we have completed the 2009-2010 school year, we are preparing for the 2010-2011 school year. All students attending the 2010-2011 school year should have received a registration packet in the mail. This packet includes 14 forms. These are comprised of both Cal State and LACHSA forms.
I have put together a few "Frequently Asked Questions" to help clarify any concerns that you may have.
Why do we have to fill out this information if we completed this information last year?
Schools need to have updated records that are accurate. Email addresses, home addressed, phone numbers do change. For the safety of every student, we want to make sure our records reflect correct information.
Why do I have to bring the registration packet in mid July on the designated dates? I would prefer to mail it in.
Many registration packets are not filled out completely. We have over 600 registration packets to enter. When the parent is attending registration, signatures, forgotten SSN and other important information can be gathered quickly. This is the most efficient way for us to collect and input the data in a timely manner.
What if we will be away during the registration dates?
If you have made plans and will not be in town during your designated time, please call the main office 323.343.2550 to schedule an appointment.
If I have my child's immunization record on file, why do I have to submit another copy?
We have one unified form that we are requesting each family to fill out. This is important for our records. NOTE: You can fax LACHSA's form to your doctor's office, have the nurse fill it out completely, have the doctor sign it, and then fax it back to you.
What is the CIN number? Is it the student ID?
The CIN number is a number issued by LACOE and used by Cal State University to identify each student. All existing students have a CIN number which follows them throughout their four years at LACHSA. Please NOTE: Ninth graders and transfer students: do not enter any number where it asks for your CIN #. We will provide this number to you at registration.
Why do we have to fill out the Cal State Forms?
LACHSA students have access to Cal State's library and health center. There are three specific forms that the university has issued to us. Please fill these forms out completely and sign. When minors are being treated at the health center or want to utilize the library a parental and/or legal guardian's signature is required.
When asked for the Social Security number should we write the student's or the parent's?
The social security number listed should be the student's.
What is the Application for Fee Waiver?
Families who qualify for the fee waiver will receive free/reduced cost lunch through our free lunch program and receive various waivers for applications including PSAT and SAT.
