Webinar: EHRs for Chronic Disease Management
Electronic health records (EHRs) have great potential for improving chronic disease management. Find out more by tuning in for this webinar, hosted by eHI, today (Dec. 15) from 3-4:30 PM EST.
Medicare Coverage for Preventive Services to Reduce Obesity
Last month, the Centers for Medicare and Medicaid Services (CMS) announced that Medicare is adding coverage for obesity screening and counseling services. According to CMS, over 30 percent of Medicare enrollees are estimated to be obese. This adds to Medicare's existing portfolio of preventive services that are now available without cost sharing under the Affordable Care Act.

Helping Smokers Quit: Tobacco Cessation Coverage 2011 Report
The
American Lung Association recently released the Helping Smokers Quit: Tobacco Cessation Coverage 2011 report. This annual report provides an overview of smoking cessation services and treatments offered in each state by Medicaid, Medicare, state employee plans, quitlines, and private insurance. The report can be used to focus media and policymaker attention on cessation coverage policies.
Check Out the New, More User-Friendly National Tracking Query System
The National Tracking Network has redesigned its layout and
developed a
new data query system to assist health professionals with finding data linking the environment to chronic diseases, such as cancer and heart disease faster. Data queries require less time to display data; information and data will be more dynamic and easier to create. Users can also sort data in many different ways, add layers to maps, and compare maps in a side by side view.
The Community Guide
The Guide to Community Preventive Services, also known as the Community Guide, is a free resource that provides detailed information on effective, evidence-based programs and policies that have improved health and prevented disease in communities across the country. Topic areas include cancer, diabetes, nutrition, obesity, physical activity, and more.

Chronic Disease Prevention Toolkit

This edition's featured tool is Health Impact Assessment: A Guide For Practice. Health impact assessment (HIA) is an emerging practice that aims to bring a greater understanding of human health consequences to public policy and decision-making. The awareness and use of HIA in the United States is rapidly increasing, and this guide aims to support practitioners in the field by describing the key tasks and activities for HIA, as well as the issues and challenges that arise in the course of practice. The guide includes illustrative examples from practice, as well as suggestions for stakeholder participation and the integration of health analysis in the environmental impact assessment process.

NACCHO's Chronic Disease Prevention Project has collaborated with national organizations and the CDC's National Center for Chronic Disease Prevention and Health Promotion in an effort to connect local and state public health practitioners with publications, tools, and other resources related to chronic disease prevention and health promotion.

NACCHO welcomes any tools or resources you would like to share with local health departments. To submit a tool to the Chronic Disease Prevention Toolkit, please contact Kelly Dawson.
