Case Manager  - Work from Home
Assume a leadership role in the multi-disciplinary care team in order to insure high quality patient care and appropriate allocation of members/patients services and resources. Conduct educational programs for all members/patients which support the continued growth of activities that are focused on measurable member/patient outcomes.   Manage care with members/patients, their families or significant others and coordinate with the PCP, SCP Social Workers, Home Care Services, Behavioral Health, CBO Intensive Case managers and other community services and all members/patients of the health care team.
                                             
                 
                   
 
Case Management - Team Leader
Training new Case Managers.   This  NYC based position requires a NY RN License and,  experience training and mentoring RN case managers. EXCELLENT computer skills, experience with Medicare and/or Medicaid case management preferred, but not required.

    Medical Record Review Specialist
Perform medical record reviews to determine quality of care, identify quality issues and assure compliance with medical record standards and regulatory mandates. You will work collaboratively with providers and educate them on all aspects of medical record review criteria.  The Chart Reviewer will also coordinate onsite reviews and communicate regularly with members, regulatory agencies etc.

Requirements: Current RN licensure, 2 plus years quality and/or related experience
