Did You Know?

In our first Healthy Village Series: Bumps in the Road, we will be learning more about preparing for and responding to an unexpected health crisis. Successfully managing life transitions enhances quality of life. Care managers can act as guides and advocates for individuals and families who are caring for older relatives or disabled adults. According to the National Association of Geriatric Care Managers, care management is the process of planning and coordinating care to improve quality of life and promote independence. The goal of care management is to improve the client's quality of life and reduce family stress through assessment, monitoring, planning, problem-solving, education, advocacy, and family caregiver coaching. Care managers can help with navigating and managing medical issues, enhancing communication between families and medical professionals, and providing referrals to community resources for housing, home care, legal, financial, and other issues. Click here to read more from the National Association of Geriatric Care Managers.
