Greetings!
 A dental implant is much like a natural tooth, which has a root in your jawbone, topped by a crown that you can see. Implants may replace a single tooth, multiple teeth, or act as support for a full denture. An implant is probably one of the most complicated procedures performed in our office requiring exquisite coordination between the dental specialist and our office. Upon determining that an implant needs to be placed the patient is referred to a specialist for an implant evaluation. The specialist will evaluate the quality and quantity of bone at the implant site, the patient's medical history, and any other anatomical structure which may impede the placement of the implant, such as nerve and sinus placement. The size of the implant, both width and length, is then determined. After the patient consents to treatment, surgery is performed by the specialist to place the implant. |

You will receive a Statement each month that reflects the Services and Payments received that month for you and your family. If you have dental insurance you will still receive a statement until the balance is paid in full. Once the insurance pays, your statement will reflect that payment and indicate the balance that is remaining for you to pay.
For Treatment that Dr. Valentine is recommending for you we will provide you with an estimate for your treatment and what your insurance will pay. Our office strives to provide the most accurate estimates. If the insurance pays more than we estimated we will refund that amount to you. However, if the insurance pays less than what we estimated you will be responsible for the balance. Financial arrangements are available. | REFERRAL TO A SPECIALIST | |
| Dr. Valentine provides excellent general dentistry. However, if you need a referral for treatment that he does not provide our office will make that referral for you. Specialists that you may be referred to include oral surgeons, endodontists, periodontists, orhthodontists, prosthodontists.
