In preparation for our interview with plastic surgeon, Dr. Gregory A. Wiener, we asked our Flashionista Board of Advisors what they want to know about cosmetic surgery. The most asked question was about non-surgical options.
(F) What are the best non-surgical options?
(W) Injections are the fastest growing segment of the industry. I can't be in business without thorough knowledge of the injectables.
(F) What's your pick in the category?
(W) I use them all, but I love Botox- it works, it's safe and it's done in 10 minutes. Botox is meant to stop wrinkles from happening by paralyzing the muscles that cause them. It's great for smoothing out forehead lines and crows' feet. Patients need to come in 2-3 times per year.
It's not a filler, so wrinkles don't disappear. But when you smile, they're not as deep or obvious; they appear as they would if your face was at rest.
(F) What will make wrinkles disappear?
(W) Fillers like Juvederm and Restylane. They're gels made with hyaluronic acid which is a natural component of skin. The hyaluronic gels are lovely because they're soft, easy to inject and last about 6 months.
Restylane has been the most popular for the past few years. I use it for nasal labial folds, to plump the lips, and for the marionette lines at the corners of the mouth. Juvederm works for this also.
Juvederm is a softer form of Restylane. I use it around the eyes, to fill in the tear trough and smooth the area beneath the eyes that becomes hollow.

(F) Any longer lasting fillers?
(W) Radiesse is a synthetic substance that contains calcium hydroxylapetite filler, which is basically the same mineral that makes up our bones. The consistency is similar to toothpaste. There are no allergic reactions associated with it. One big advantages of Radiesse is that it can last up to 3 years. It's used for lip augmentation.
Artefill is the only FDA approved permanent filler. It's actually microscopic spheres of a plastic-like collagen substance. You have to be careful with Artefill because it can trigger an inflammatory response and cause scarring. It's being marketed for cheekbone augmentation and liquid facelifts, where you actually fill the face to restore volume. I use it for the nasal labial fold.
Sculptra is more forgiving than Artefill and can be used all over the face. It's made from similar materials used to make dissolvable sutures. Sculptra is injected in a grid-like pattern. It's labor intensive, as each cheek requires 30 or so small injections and anywhere from 3-5 treatments before seeing real results but it lasts 1-2 years. The body tissue binds to the material and causes a plumping effect to the skin.
