Dear Friends of At Home Solutions:
As we embark on a New Year, one of my goals is focusing on being prepared and having a back-up plan ready. You never know what's going to come your way!
I think it's important to remember the three crucial documents each of us needs completed. I always say this paperwork is ABOUT us but it's really FOR our loved ones who are left to make the important and sometimes difficult decisions on our behalf.
The most important legal papers everyone over the age of 18 should have:
1. Health Care Directive: describes what your medical wishes are and appoints trusted friends / professionals to carry them out (when you're not able to yourself)
2. Power of Attorney: describes what your financial wishes are and appoints trusted friends / professionals to carry them out (when you're not able to yourself)
3. Will / Living Trust: designates where your real and personal property go after you pass on and designates who makes sure your wishes are carried out. (Important to know: Trusts help you avoid probate - where your finances are public record and can be researched by those motivated to do so.)
NEW: POLST ( Provider Orders for Life Sustaining Treatment) established 2009 in Minnesota allows loved ones, medical professionals and home care providers to directly carry out your loved one's wishes who have serious illnesses. This document puts control directly into the hands of the patient to ensure their desires at the end of life are followed through in more detail. I earned my Bachelors Degree in Psychology from San Francisco State University. While in San Francisco I had an internship at the Institute on Aging, an organization that promoted the ethical treatment of seniors in the Bay Area.
I chose working with older adults because I feel their needs are under represented in the community. I started working with At Home Solutions in October of 2009 and still love it to this day. When I am not working I enjoy reading and spending time with my brand new baby niece!
