Congress Puts Off Cuts to Doctor Medicare Payments

WASHINGTON (AP) - Congress agreed Monday to a one-month delay in Medicare payment cuts to doctors, giving a short-term reprieve to a looming crisis over treatment of the nation's elderly.
The House, in approving by voice vote the bill passed by the Senate earlier this month, postponed a 23 percent cut in doctors' pay scheduled to take effect Dec. 1. That gives lawmakers a month to come up with a longer-term plan to overhaul a system that in recent years has bedeviled Congress, angered doctors and jeopardized health care for 46 million elderly and disabled.
"This bill is a stopgap measure to make sure that seniors and military families can continue to see their doctors during December while we work on the solution for the next year," said Rep. Frank Pallone, R-N.J., chairman of the Energy and Commerce health subcommittee.

NANS SYMPOSIUM 2010 SHOWCASES INTERNATIONAL LEADERS IN INTERVENTIONAL PAIN .
Last weekend I attended the NANS (North American Neuromodulation Society) meeting in Las Vegas, NV, held at the Encore Hotel. The meeting was well attended and well received by the more than 600 participants. I was excited to be an exhibitor and very pleased to meet and greet Hartley friends, colleagues and clients who visited our booth in this, our fifth year of attendance.
I was awed by some of the outstanding presentations by leaders in the interventional pain community - exploring topics ranging from intrathecal drug delivery to spinal cord stimulation. I particularly enjoyed the Saturday lectures on intrathecal therapies delivered by Dr. Elias Veizi and Dr. Gladstone McDowell.
Veizi's Outcomes of Intrathecal Trial with Dual Agents in Failed Back Surgery Patients presented a fascinating study examining the difference in pain reduction effectiveness when using opioid with anesthetic vs. opioids alone.
Dr. Veizi showed that over a period of time, when Bupivacaine was administered in combination with an opioid, the two agents proved superior in treating chronic pain. The result was a reduction in required dose escalations as opposed to the rapid increase demanded when using just a single opioid.
Dr. Gladstone McDowell presented The Use of the Personal Therapy Manager with Prialt for Patient Controlled Analgaesia: Case Theories Highlighting Techniques and Outcomes.
His findings were the result of a year-long study which involved the treating of patients with Prialt and the utilization of the Personal Therapy Manager (PTM). A unique feature of the Medtronic Pump, the PTM allows a patient to personally administer a pain-relieving drug throughout the course of a day via hand-held device. In the event of breakthrough pain, the PTM facilitates communication between device and pump to address the need for additional pain medication.
Side note: On the second day of the symposium, I had the opportunity to meet three Canadian physicians at our NANS booth. They have asked me to speak at their upcoming national meeting in June. I'll fill you in on the details in future issues of The Hartley Standard.

| PHARMACOLOGY REVIEW: MORPHINE CONVERSION TO FENTANYL | |
Morphine is considered the gold standard of therapy for treatment of acute and chronic pain. In regards to intrathecal drug therapies, most physicians will trial their patients on morphine and subsequently institute appropriate measures.
In conditions where morphine is not acceptable or is not yielding the desired clinical effects, clinicians will consider changing to fentanyl citrate. When doing so, it is important to carefully consider both potency and characteristics.
Fentanyl differs from morphine both in regard to clinical response as well as pharmacological characteristics. Morphine is hydrophilic, and fentanyl is very lipophilic, and there is the issue of potency. There is concern among clinicians about changing from morphine to fentanyl because of the fact that fentanyl is demonstrably more potent: in acute pain 100 times more potent and 50 times more potent for chronic pain than morphine.
Due to fentanyl's lipophilic properties, the distribution of this drug is more focal as related to the catheter tip. Tissues within the intrathecal space are very (dense in fat, fat saturated, fatty), with a great capacity for absorbing fentanyl, consequently, fentanyl does not travel significantly within the intrathecal space.
Once the decision has been made to change therapies, I recommend a 1 to 25 conversion rate. Although the "textbook" conversion recommendation for chronic pain is 1 to 50, I consider that to be too conservative, based upon the comparative characteristics of the drugs themselves. A 1 to 25 rate is still conservative...but I strongly suggest that physicians achieve their own comfort zone as they gain experience with the conversion process. And, of course, weigh the individual differences and concerns of each patient when determining what conversion level will best serve. |

A Los Angeles native, Stacy came to Hartley in 2004 with a background in pharmacy tech and a PTCB certification. In two years, she was promoted from Distribution Coordinator to Executive Assistant, and in two more years she rose to her current position. She is now responsible for managing all aspects of office organization, productivity, and administrative support - including (but not limited to) Human Resources, Accounting, Production, and Motivation.
That last one is key, because Stacy is that rare individual tasked with ensuring that employees are motivated to help customers and address their needs, while also ensuring that Hartley clients remain Hartley clients.
At Hartley we have chosen to hang our collective hat on consummate customer service. Stacy sees to that at all levels. She is the poster woman for multi-tasking and every-stage accountability. She's a good listener, a quick study, a trusted source of strength for all to draw upon...and she knows how to make the decisions that have to be made.
