Greetings!       

Changes continue in the field of on-site clinics as big corporations involved affiliate, acquire and generally press forward.  Walgreens acquired Duane Reade and chose Greenway Technologies' EMR for all its clients; Novia CareClinics raised $1M in capital for expansion; and technology is king with the adoption of Dossia by Intel and Pitney Bowes.   Read more about all these issues on our Web site at www.onsiteclinics.org between newsletter editions.  We are also noticing an uptick in employment ads by vendors and consulting groups that inhabit the on-site scene.
Value Purchasing and Narrow Networking
This is probably an idea that is searching for an acronym.  It is certainly not new;  it is just being rediscovered by employers.  Once the employees elect and select an on-site physician, the question of referrals arises and someone wonders about being selective.  Some of the pioneers in the field, like QuadGrapihics, have been doing the math (and the contracting) for over a decade.  "Narrow Networking" or "Value-based contracting" has long been one of the central features of mature, employer-managed healthcare programs ensuring access, cost savings, and quality as employers contract with specific hospitals, specialists, and other providers (hand-picked because of their ability to work closely with the on-site clinic primary care component) to keep costs down and drive quality. 
 
But, how did Lowe's figure this out?  Well, they did the same thing that the number crunchers do for their on-site clinic bosses/clients.  They studied the data.   Lowe's recently announced that it has reached agreement with the Cleveland Clinic to be the primary provider for heart procedures for all Lowe's employees (employees will have a choice, but will have reduced out-of-pocket costs and travel expenses paid if they use the Cleveland Clinic).  In exchange for this funneling of volume, the Cleveland Clinic has agreed to reduce unnecessary procedure volume and drive up the stats on outcomes for the procedures that they do perform.
Intel & Pitney Bowes Offer Dossia to Employees
We encourage you to go to the Web site, www.dossia.org, to find out about this not-for-profit corporation (funded by employers) and its goal of providing  ". . . a secure data infrastructure that aggregates and stores health information for individuals to create a lifelong personal health record with medical information from multiple sources.  Most importantly, Dossia's health record is personally-controlled, private, and portable."
 
The dream is to have employees have available to them in a hyper-secure manner, a life-long medical record that encompasses all their medical history.   Intel and Pitney Bowes are founding members of  the corporation and are rolling out the Dossia platform to employees now and throughout 2010.   Both corporations already have on-site clinics.  Obviously, they believe this will be another important tool for integrating healthcare data, reducing costs, improving quality, and  supporting and increasing utilization of wellness programs for those employees who sign up to use the Dossia application.
 
One wonders if Microsoft and Google are watching.  They both have offerings that complement this type of a process, although they are focused on the employee and consumer rather than the employer.  Their business models anticipate ad revenues but the orientation of the DOSSIA approach is population health management.
