I have received confirmation from Harry Manley that Highmark is indeed limiting modalities to four (4) units/procedures per day.  They will choose the 4 highest paying codes to process and pay. New Medical policies Y-9 and Z-6 will be updated as of September 1st to reflect this change.  At this time it is not in writing but Harry is going to try to send something over to MBSS as soon as possible. 

It has been confirmed that authorizations are needed if Highmark is the secondary payer.  In addition we have also confirmed that out of state policies DO NOT need the authorization.
