| TransforMED and the American Academy of Family Physicians - Stronger Together | |
It could be the Lone Ranger and Tonto or Butch and Sundance, but whatever the analogy, TransforMED and the AAFP are major players in the drive to have the value of primary care fully recognized. Each organization individually is a force for primary care, but together their strength and credibility is unrivaled. The AAFP founded TransforMED in 2006 with $8.5 million dollars out of reserves because it believed in the future and critical importance of Family Medicine to the US healthcare system. They also understood that Family Medicine and indeed primary care needed to change if it was going to survive.

| | NEW: PCMH Care Coordination Workbook
In the PCMH model, primary care practices guide and advise their patients beyond the practice setting, often through cooperative alliances and structured relationships. Care coordination expands the definition of team to include all providers, locations and resources necessary to meet the patient's healthcare needs - specialists, hospital facilities, home health facilities, pharmacists, social workers - any healthcare entity that affects the patient's healthcare.
Effective care coordination provides for an understanding of each team member's role and responsibility at the time of the patient interaction and a clearly defined flow of accurate information between team members. Click here to buy the book
