New Evidence: Facilitation Improves Practice Transformation to New Models of Care

RGC Logo! The Robert Graham Center has released a new one-pager entitled "The Effect of Facilitation in Fostering Practice" by Carlos Roberto Jaén MD PhD, et al. The report summarizes the researchers' findings that "Working with facilitation agents measurably improves the ability of motivated primary care practices to move towards improved models of care."

CareFirst BlueCross BlueShield PCMH Pilot Project

The 2-year project includes internal medicine, family practice, and pediatrics practices and will explore what difference the PCMH model makes for improving outcomes and increasing reimbursement.

TransforMED & Priority Health Conduct Learning Collaborative
The learning collaborative gave the primary care practices participating in the Priority Health pilot a chance to interact with colleagues from other practices, share common experiences and offer perspectives on tackling the challenges ahead.

"The burden on primary care offices will double in the next five to seven years," said Priority Health's Chief Medical Officer Jim Byrne, M.D. "If ever there was a reason to take on the patient-centered medical home that is it."
