"Help when I needed it"
An (unfortunately) true story

A few too many minutes on hold and a two-week wait for an appointment to address a UTI doesn't meet a suffering patient's need and ends up driving her into the arms of a CVS Minute Clinic. This personal narrative highlights the nexus between patient-centered care and advanced access and clearly illustrates why implementing the TransforMED Model of Care advanced access scheduling component is a critical aspect of providing patient-centered care.

Commonwealth Fund grant finances patient-centered survey and access to Web training for NDP practices

The Commonwealth Fund has awarded a six-figure grant to family medicine researchers at the University of Texas Health Science Center who are working with TransforMED to test innovations in patient-centered care. The grant will be used to survey patients served by NDP practices to learn about their access to health care services, engagement in health care decisions, access to the practice's information systems, care coordination and views on how well the practice teams work together.

The grant also provides NDP physicians free access to an interactive, web-based learning resource for healthcare communication published by the American Academy on Communication in Healthcare (AACH). Forty multimedia-rich interactive on-line modules and hundreds of videos show communication competencies considered essential by a recent Institute of Medicine report.
