DIRECTOR - Rural Health Medical Home Initiative
Salary Range: $60,000-$70,000

Hire a Director to oversee the development of Community Health Centers in identified rural communities in preparation for the establishment of new FQHC/ Community Health Centers. Centers will be prevention and wellness-based and offer comprehensive disease management programs.

In order to assure that all members of rural communities receive the healthcare services they require, the Rural Health Medical Home Initiativeis working with healthcare providers to form a system of care for those patients who have low or no income and are without healthcare funding.

The Rural Health Medical Home Initiativeworks with local community service providers to meet the healthcare needs of the community through a collaborative coordination of available resources and services. The Director of the Rural Health Medical Home Initiative will work to develop strong, streamlined relationships with outside providers to ensure patients receive the highest level of care available.

Job Summary
The Director of the Rural Health Medical Home Initiative shall work with partners and community leaders to facilitate the development of Community Health Centers in identified rural communities in preparation for the establishment of new FQHC/ Community Health Centers that are prevention and wellness based and offers comprehensive disease management programs. The position will also be responsible for communicating relevant information to the community partners overseeing the proposed system of care in order to continually improve the coordination and availability of services provided in the community.
Competencies will include extensive knowledge of the health care industry, issues, payor mix and trends, disease management models, market research, environmental scanning, identification of resources and opportunities, proposal design and management, development of business partnerships and relationships and marketing, and have a demonstrated record of success in leading a business development unit to win federal/state/private contracts.
Primary Duties and Responsibilities
The Director of the Rural Health Medical Home Initiative shall be responsible for:
- Leading the design and execution of a non-profit or collaborative-based community health clinic with a disease management focus
- Identify central eligibility systems
- Implement a system to follow up with patients to make sure that aftercare is obtained as instructed.
- Establish performance metrics related to the success of Rural Health Medical Home Initiative and monitor these
- Developing business plans for new corporate services/products
- Managing competitive/ industry research and analysis to yield strategic insights and strategic business alternatives based on industry trends.
- Identifying and qualifying new business opportunities - primarily federal and state agencies dealing with health, healthcare, quality improvement, and fraud/waste/abuse of healthcare services.
- Interacting, leading and coordinating with all members of the Proposal Team and Business Development team for proposal and other product production efforts as required. Identifying and establishing priorities related to community healthcare resource availability and utilization.
- Work with healthcare providers in the network to improve information technology that allows sharing of information to better serve patient care needs.
- Work closely with local healthcare clinics and other healthcare providers to continually evaluate the value and availability of healthcare resources in the community
- Work collaboratively with other members of the healthcare access network
- Develop and provide education to external constituents and internal staff regarding the Rural Health Medical Home Initiatives.
- Establish working relationship with the community, and various agencies to promote the development of health promotion and prevention.
- Consistently maintain confidentiality according to HIPAA.
- Follow established federal, state, and/or local policies, procedures and guidelines relating to health and safety.
Job Qualifications
A Bachelor's degree or Master's degree in healthcare, human services, nursing or business preferred and/or relevant experience is required. 5-7+ years managerial experience in a healthcare organization, preferably a community healthcare setting is expected; must be well organized, detail-oriented and have strong analytical skills.
Must be able to demonstrate effective verbal and written communication in all situations with all levels of staff, management both internally as well as externally. Must have excellent interpersonal skills. Must be able to work independently with minimal supervision. This position requires a strong knowledge of Microsoft Office software including Word, Excel and PowerPoint.
