What is the Capital Coalition for Health?

 

The Capitol Coalition for Health is a team of leaders from communities across Leon County that have an important role in the local public health system. Using the MAPP process, the Coalition will work together to identify areas of need and opportunity toward the shared goal of improved community health in Leon County.    
Our Vision
Leon County communities will distinguish themselves as collaborative, civically-engaged and accountable.  These will be communities where residents and leaders support health-based policies that ensure the healthy choice is the easy choice; where residents can live, work, and play in a safe and healthy environment; and where every resident has   access to affordable and equitable health care resources.

Our Values

Leon County:  

  • Will invest in the health and well-being of all residents
  • Will model healthy and active lifestyle
  • Will advocate for equitable and transparent health-based policies 
How Can YOU Get Involved?
  • The Community Themes and Strengths Subcommittee guides plans and outreach to assure that the voices of residents from all communities are included via a community survey and focus group discussions. CTSA subcommittee will meet for the first time via webinar on Oct. 7.     
  •  The Local Public Health System Assessment subcommittee identifies entities that provide public health services and assists in engaging these entities in discussions about the assets and gaps in services.  The LPHSA subcommittee will begin their work later in October.
  •  Sign up for our e-newsletter for updates. This will ensure that you receive the electronic link to the survey! ***If you received this email directly, you are already signed up for the newsletter. 
  •  Participate in the Community Event in May where the Capital Coalition for Health will report findings and plans developed during the MAPP process and launch the Action Cycle to improve community health!  More information about this event will be forthcoming in 2012.

MAPP
What is MAPP?

 

Mobilizing for Action through Planning and Partnership (MAPP) is a community-driven strategic planning process for improving community health. Facilitated by public health leaders, this process helps communities apply strategic planning to prioritize public health issues and identify resources to address them. MAPP is not an agency-focused process; rather it is an interactive process that can improve efficiency, effectiveness, and ultimately the performance of the local public health system.  

 

The Four MAPP Assessments

Each of the four MAPP Assessments provides new information about the assets and needs of the community

 

1. Community Health Status Assessment includes a comprehensive review of demographic and health status data, including analysis of health disparities among subpopulations. This assessment answers the questions:

             How healthy are our residents?

             What is the health status of our community?

 

2. Community Themes and Strengths Assessment gathers input from local residents through surveys and focus group discussions to answer the questions:

What is important to our community?

How is quality of life perceived?

   

3. Local Public Health System Assessment employs a consensus-building process to identify the strengths and gaps within the broad public health system by reviewing the Essential Public Health Services. This leads us to answer:

             What is the capacity of our system?

             How are needed services provided in the community?

 

4. Forces of Change Assessment identifies key forces and describe the associated impacts related to health and quality of life. This assessment answers the questions:

What is happening or might happen that affects the health of our community?

What are the threats or opportunities?

The Plan and Timeline

 

The Leon County MAPP Process began with mobilization of community partners and creation of a shared vision for the health of the community. With oversight and direction provided by the Capital Coalition for Health MAPP Steering Committee, the process will continue as follows:

 

Four MAPP Assessments:  

September 2011- January 2012

Identify Strategic Issues:  

January - February 2012

Formulate Goals and Strategies:  

March - April 2012

Community Event to begin Action Cycle:  

May 2012

Leon County Health Department sponsors this community effort, in collaboration with Health Planning Council of Northeast Florida, which serves as the neutral process facilitator.
