BACKGROUND AND PURPOSE: The Health Planning Council of Northeast Florida and Healthy Jacksonville are preparing to submit an IMPLEMENTATION proposal to the U.S. Centers for Disease Control and Prevention (CDC) for a Community Transformation Grant under funding opportunity number CDC-RFA-DP11-1103PPHF11. You can obtain a copy of the full CDC FOA at Grants.gov. With this Request for Brief Partner Proposals, the Heath Planning Council and Healthy Jacksonville are seeking qualified partners to support work in five Strategic Areas and participate in the development of its grant application to the CDC. While the CDC indicates that multiple applicants serving populations within the same geographic area may apply for funding, the CDC will fund only one application within the same geographic area.
Under this announcement, the Health Planning Council and Healthy Jacksonville are seeking funding support for a Duval county-wide policy, environmental, programmatic, and infrastructure changes under each of the following Strategic Areas:
- Tobacco-free living -Tobacco Free Jacksonville
- Active living and healthy eating -Healthy Jacksonville
- High impact evidence-based clinical and other preventive services (specifically prevention and control of high blood pressure), -Front Porch Collaborative
- Social and Emotional Wellness, -Safe & Healthy Duval Coalition
- Healthy and safe physical environment - Health Planning Council
The county-wide application will also address additional "Healthy People 2020" focus areas that directly relate to the overarching goal of reducing the incidence and prevalence of chronic disease, and are aligned with the strategic directions listed in this grant solicitation, including: arthritis and osteoporosis; cancer; diabetes; disabilities and secondary conditions; environmental health; HIV; mental health and mental disorders; and substance abuse. The Health Planning Council and Healthy Jacksonville will incorporate the overarching "Healthy People 2020" goals in plans and strategies. These include:
- Attain high quality clinical preventative services
- Longer lives free of preventable disease
- Reduce long-term disabilities
- Injury prevention
- Premature death
- Achieve health equity
- Eliminate health disparities, and improve the health of all groups
- Create healthy and safe physical environments that promote good health for all
- Promote quality of life, healthy development and healthy behaviors across all life stages
The CDC has issued a list of evidence- and practice-based strategies that may be included in applications. These strategies are attached; please note that strategies identified in bold font are required and will be included in the application. You can also seek additional practice-based strategies on Northeast Florida Counts [www.neflcounts.org].
With this request for Request for Brief Partner Proposals, the Heath Planning Council and Healthy Jacksonville are seeking qualified partners to support work in each of the above areas and participate in the development of its grant application to the CDC.
REQUESTED RESPONSE: Responses to this announcement should address the following areas:
- Clearly state which of the five Strategic Areas you are proposing to address [Tobacco-free living; Active living and healthy eating; High impact evidence-based clinical and other preventive services (specifically prevention and control of high blood pressure); Social and Emotional Wellness; Healthy and safe physical environment]
- Describe your relevant past experience in implementing policy, environmental, programmatic or infrastructure change.
- Briefly summarize proposed staffing required to implement the identified strategies.
- Provide a one page profile identifying your organization's mission, primary activities, any grants you have received within the past four years that is prevention focused and addresses Healthy People 2020 goals, and staff leadership team.
- Provide a cover letter with key contact information, to include the applicant's point person for this opportunity, with phone number and email address. The contact person should be available for several in-person and telephone meetings through July 10th. Please include 2 or 3 organizational references.

REVIEW PROCESS
All submissions will be assessed for responsiveness by a panel of reviewers.
Agencies selected to participate in the application development process will be notified no later than June 13th.
The Health Planning Council and Healthy Jacksonville will retain the ability to select one or more partners for each Strategic Area
GUIDELINES FOR RESPONSE
Responses should be no more than the cover letter, the one page organizational profile, and a 4-page narrative response to items 1-4 under the Requested Response Section double-spaced pages in 12 point font.
Responses are due no later than 4:30pm Thursday, June 9, 2011.
