Baker County Health Assessment Release
The Baker County Health Department, Healthy Baker, Inc. and the Health Planning Council of Northeast Florida are hosting a presentation on Monday, April 11, 2011 to present the Baker County Health Assessment report to the Baker County community. The press conference will be held at the Baker County Health Department and will start promptly at 11:45 AM.
Healthy Baker, Inc. represents an ongoing partnership between the Baker County Health Department (BCHD) and a coalition of key community stakeholders from around the county who collaboratively work toward a mission to improve the health of Baker County residents by focusing efforts on multigenerational prevention and health promotion activities that identify, coordinate and promote health resources within the community.
"At a time when our nation is facing critical health care issues, people are becoming much more aware of the impacts of lifestyle, physical and economic environments on the health of the community. The Health Planning Council's report gives the local organizations involved in providing health care services to the people of Baker County a better understanding of the community's health care concerns and needs, as well as providing a plan of action to help address those needs," stated Kerry Dunlavey, Administrator for the Baker County Health Department.
The Health Planning Council (HPC) served as the convener for the community stakeholders participating the process. The HPC also helped collect and analyze the data throughout the assessment. Significant key issues and themes were recorded and updated throughout the process based on empirical evidence and community discussion. Key issues were then consolidated and prioritized based on the scope and severity of need, as well as the availability of resources to address them. The partnership then worked to develop appropriate and realistic goals and strategies pertaining to each priority issue. This report presents a summary of the key health issues along with recommended goals and strategies developed through this process. The Healthy Baker consortium will work in close partnership with BCHD to ensure continued progress toward the goals listed in this report.
This assessment and plan is the product of a collective and collaborative effort from a variety of dedicated health and social service providers along with other invaluable community stakeholders from across all regions of Baker County. It is hoped that the findings from this community health assessment and plan will serve to guide the Healthy Baker members, as well as all health and social services providers in the county, in their program development efforts over the next 3-5 years. This health assessment will also help the Baker County Health Department in preparation for accreditation.
The Health Planning Council of Northeast Florida has been addressing the healthcare needs of Baker, Clay, Duval, Flagler, Nassau, St. Johns, and Volusia counties (Region 4) for more than 40 years under a variety of federal, state, and local health planning initiatives. We strive to meet Florida's healthcare challenges through planning, research and evaluation initiatives that target local healthcare needs and affect health policy.
