On behalf of the Duval County Immunization Task Force and the Duval County Health Department, you are cordially invited to attend a 2009 H1N1 Pandemic Flu and Seasonal Flu Wrap Up meeting on June 9, 2010 from 11:30-3:00 at the University Center on the campus of University of North Florida.

The purpose of this meeting is to discuss specific strengths and areas for improvement of the public health and community pandemic response. Each agency and partner's experience during the pandemic is critical to corrective action and improvement planning for Duval County and its surrounding counties.

Additionally, this invitation is accompanied by two short, 10-15 minute, surveys to gather information relevant to your experiences. One survey is for Emergency Preparedness Partners (EMS, hospitals, law enforcement physicians, nurses, clinics, health departments, school health, PIO's, etc..) and the other survey is for Community Partners (churches, coalitions, non-profit organizations etc...).  Please make every effort to complete and return the survey to the Health Planning Council of Northeast Florida by June 3, 2010. This will improve the efficiency and flow of our meeting.
