"In Over Your Head
with a Difficult Extraction"
Protocol # 41 (Excerpt from "The Protocol Book")
 This is a clinical protocol. If you're struggling with an extraction over 45 minutes... bail. Get out the chromic. Place some gauze. Give post-operative instructions and a pain script and send the patient to the oral surgeon. Tell them:
"Listen, this tooth is more challenging than I anticipated. I'm going to place a stitch, give you a pain script and send you to my friend, Dr. King."
Be calm. Act like "these things can happen" and waive any fee.
Rationale
Ever start a tooth you had wished you hadn't? I wish I had had someone tell me this one year ago. Listen, I'm not an oral surgeon but I did stay at a Holiday Inn last night. Ok, enough kidding. I called an oral surgeon, a malpractice attorney dentist and used my own expertise. This is what we concluded:
First, remember that if you tell the patient this one is for the oral surgeon and the patient talks you into it, you are held to the oral surgeon's standard of care, which includes not leaving behind root tips. Forgetting all of this: if one day you find you're in deep, give the same post-op care as if you took the tooth out and remember that it's your responsibility to find them an oral surgeon - hopefully one that likes you. The point is, don't do what I once did. Don't cancel your afternoon, keep them in the chair three hours claiming victory over a battered, barely breathing patient. Forty-five minutes is tops. If it ain't out, get out.
