Protocol # 79 (Excerpt from "The Protocol Book")
This is an intangible protocol. It beckons to the doctor's philosophy of care. It says that when possible, recommend to a patient who just had recent work, who may have completed a quadrant of care last year to take a breather - a break. Even when they have obvious need (not pain) for further care. Tell them at the recall:
"You know Jane, you might remember at our first visit I mapped out a few different phases of work that we'll need to do to get your mouth in great shape. And we'll eventually get you done. But, I don't see any reason why we can't give you a pass this time around. If anything in the next quadrant kicks up on you, let me know. I'll be here."
Rationale
To the clinicians: read this and the re-read it. There is no more important protocol for your long-term success. When you practice comprehensive care, you have to face reality. Most patients are not used to the approach, time, and money that come with complete dentistry. You can embrace the highest integrity; some patients will question the care of asymptomatic pathology. Some will think you're only lining your pockets.
If you walk your talk, if you do a good job and they return for recall, you'll take a giant leap toward having a long-term patient with this approach.
Get over yourself. They likely won't expire if you hold off a little on the next restorative phase (obviously, there are times like opening the vertical when the timing of the next phase is an issue. Like when you place centric stops with composites that call for a more rigid material in a patient's occlusal scheme.)
So even if it's a "weak week," and it would be really nice if this still happy patient would commit to phase 2, hold off, unless they unequivocally ask to continue.
Although this patient may not contribute to next month's productivity, the trust you earned will be an enduring force of future accepted plans for years to come.
