Saint Joseph's East Targets Core Measure for Pneumonia Antibiotic Administration Performance Measure Driven from 46% to 100% Compliance
"We are excited by the early outcomes we are achieving through this
innovative use of the GetWellNetwork System and look forward to
achieving optimal care delivery surrounding this process." - Eric
Gilliam, Administrator, Saint Joseph East
With anticipation of Pay for Performance, hospitals have placed priority focus on those Core Measures with greatest variance from the Top Hospital norms. Under the leadership of Eric Gilliam, Administrator, Saint Joseph East in Lexington, KY employed a GetWellNetwork® Patient Pathway™ to target their performance on the Pneumonia Antibiotic Core Measure.
Situation. It has been proven that timely administration of antibiotics can improve outcomes for pneumonia patients. The Department for Health and Human Services (DHHS) recommends administration of antibiotics within four hours of arrival. Adherence to this measure is associated with decreased mortality, a decreased length of stay, and lower cost per case. Historically, at Saint Joseph East, 46% of pneumonia patients received antibiotics within the recommended time period.
Solution. Eric Gilliam saw the GetWellNetwork technology as a tool to address this compliance issue and, along with the GetWellNetwork Professional Services Team, worked to design and implement a custom Patient Pathway. The Pneumonia Antibiotic Pathway went live October 29, 2007.
Application. The GetWellNetwork PatientLife System® receives a trigger from the hospital's Admission, Discharge and Transfer (ADT) system upon admission of a pneumonia patient. An on-screen alert is sent to the patient within three hours of admission asking the patient if they have received an antibiotic. Notification of the response is sent to Nursing for appropriate, timely follow-up.
Outcome/Results. Since the launch of the Patient Pathway, the compliance rate at Saint Joseph East for this performance measure has risen from 46% to 100% and has remained consistent for the past two consecutive months. This creative initiative is an exceptional example of engaging patients in their own care and in turn driving outcomes for the hospital. The Pneumonia Antibiotic Pathway is now available and generally used by GetWellNetwork clients.
