Currently, the management of patients with Alzheimer's Disease involves drugs that provide symptomatic relief.  The Division of Geriatric Medicine, St Louis University Health Sciences Center reports that research is focusing on disease modifying and/or preventive approaches.
 
    Two classes of medications are currently approved by the FDA for the treatment of Alzheimer's disease:
· Tacrine, donepezil, rivastigmine, galantamine- mostly for mild to moderate Alzheimer's Disease
· Memantine- for the moderate to severe stages of Alzheimer's disease
 
    Management of multiple medical problems can be more complex in patients with dementia. Patients with Alzheimer's disease are often unable to follow instructions to properly take their medication, but compliance is essential for attempting to slow disease progression and improve or stabilize quality of life. "Simplifying treatment regimens and providing more caregiver- and patient-friendly modes of administration that fit in better with daily routines can ease caregiver stress which, in turn, may have a favorable impact on the patient's condition. To overcome problems of medication adherence in the elderly, simple, user-friendly dosage regimens should be prescribed for all medications..." There is a need for novel treatments for Alzheimer's disease, such as once-daily donepezil, memantine and galantamine. Transdermal medications include creams and gels that allow medication to be absorbed through the skin, and rivastigmine may be a medication that can be delivered transdermally.

 Many people experience problems with dry nasal mucous membranes, a condition which is worse during the winter, in warm and dry inland climates or deserts, and after the nose has received radiation therapy. Nasal mucosa dryness may result in a "bloody nose" and can also be caused by low humidity during long journeys by airplane and in air-conditioned environments. Normal saline nasal spray (isotonic sodium chloride solution; ISCS) has long been the most commonly used therapy for nasal dryness, but use of normal saline actually results in little change with regard to the nasal mucosa (lining of the nasal passages). Pure pharmaceutical grade sesame oil (NF) has been shown to be significantly more effective than normal saline for treatment of dry nasal mucosa and nasal stuffiness.
 
   Pure sesame oil (NF) in a metered spray bottle, administered as 1 to 3 sprays in each nostril 3 times daily,  has been shown to significantly decrease nasal dryness.
 
   Sesame oil contains a high concentration of tocopherols (vitamin E) that might neutralize oxidants and prevent tissue damage and inflammation that would otherwise occur.
