Comforting Patients Nearing the End of Life:
Topical Medications to Relieve Pain and Nausea & Vomiting
The goal of palliative care is the achievement of the best possible quality of life for patients and their families by providing relief from pain and other distressing symptoms.
 
Pain Management
 
Effective pain management is best achieved by a team approach and individualized therapy. Besides relieving suffering, pain control is crucial because even when the underlying disease process is stable, uncontrolled pain prevents patients from taking pleasure in their usual role in the family and society.
 
Optimal therapy may require the use of a combination of medications that work in different ways to relieve pain. Patients with cancer and other forms of chronic disease may develop problems swallowing, or have nausea and vomiting or other gastrointestinal problems that make it difficult to take oral capsules or tablets. Our compounding professionals can help by formulating solutions or suspensions flavored to please the patient, or by compounding the needed medications into a topical or transdermal gel or cream that allows the medicine to be absorbed through the skin.  Relief of Nausea & Vomiting
 
Chemotherapy-induced nausea and vomiting (CINV) is commonly cited by patients as being among the "most unpleasant and distressing" side effects associated with chemotherapy. Persistent nausea can often be effectively controlled by using a combination of medications tailored to meet each individual's specific needs.  A combination of lorazepam, diphenhydramine, and haloperidol (known in combination as "ABH" or "ABHR" when the medication metoclopramide was added) has been prepared as a transdermal gel and a rectal suppository, and found to be effective in the majority of hospice patients with minimal side effects. However, patients with bowel obstructions developed problems when treated with metoclopramide.
