We were pretty excited about the Cross Settings III BPIP release. If you haven't downloaded this package, definitely do so today! It's packed full of innovative ideas. Here is just a peek at what you'll find in store.
  • Does home care have a role in Accountable Care Organizations?  Download the Cross Setting III BPIP and read pages 21-24 to find out!  "Since ACOs will be looking for partners to achieve this goal in the most effective and efficient manner, home health agencies must aggressively pursue these groups and convince potential ACOs that they can provide services needed for goal attainment."  Mary St. Pierre, CS III BPIP, page 23
  • What is a provider community? Can you identify your community of providers? Download the Cross Setting III BPIP and read pages 25-26 for an explanation of a provider community. HHQI Joins National Study on Reducing Unplanned Hospitalizations


Everyone in home care knows the story, each year millions of patients are discharged from hospitals only to end up right back in the emergency room. How can we remedy this financially draining problem? Delta Health Technologies, LLC is teaming up with the Home Health Quality Improvement (HHQI) National Campaign, the National Association for Home Care & Hospice (NAHC) and Fazzi Associates will work to identify best practices in reducing unplanned hospital readmissions of Medicare Patients in home care.  

The study's inital report of findings will be presented at NAHC's 30th Annual Meeting in Las Vegas in October 2011. A complete study will be available in late October. The study will also provide webinars at no cost to home care agencies.

The first panel discussion will be held July 12 and will include two of HHQI's very own, Eve Esslinger - HHQI Lead RN Project Coordinator and author of the BPIPs and Jill Manna - HHQI Lead Data Analyst. For more information, contact us at hhqi@wvmi.org.
