HHQI Campaign Continues

As first reported in the CHAMP Community Blog, the Home Health Quality Improvement (HHQI) National Campaign will continue after July 31, 2011. The campaign's second phase was slated to conclude on this date, but the Centers for Medicare & Medicaid Services (CMS) have indicated that the HHQI movement will persist into a third phase and carry on offering free quality improvement resources at www.homehealthquality.org.

 

Details on the structure and timeline of the Campaign's next phase are not yet available, but will be shared as soon as possible. In the meantime, please share your input on how the campaign can improve by filling a brief three-question assessment that asks for your insights, along with your name, e-mail address and phone number. You should be able to complete it in about a minute at: http://www.homehealthquality.org/hh/about/feedback/default.aspx

Reward Hard Work - Go for the Gold!

 

 

Reward your hard work in this campaign!  

 

The cutoff date to be part of this HHQI Rewards program is June 17. Special gold, silver and bronze medals denoting engaged campaign participation will be distributed all eligible HHQI National Campaign participants via email by the end of the month.


To be eligible for the gold medal, make sure you have downloaded the last four Best Practice Intervention Packages (BPIPs) from www.homehealthquality.org. Doing so is simple. Just follow the three steps below.  
 

 

1. Log in to the HHQI site with your username and password here

2. Click on "My Links" in the gold toolbar

3. Click on the name of the BPIP(s) you would like to download:

 

  • Fall Prevention
  • Cross Settings I
  • Cross Settings II
  • Cross Settings III

You have until June 17 to make sure you have downloaded these four BPIPs to get the gold. Once you receive your electronic medal, you may wish to use it on your Web site, in your newsletter or anywhere else you would like to display your agency's commitment to quality.

 

Here is just a taste of the information you'll receive from these fantastic resources to reduce acute care hospitalizations!

 

  • Do you know the Seven Essential Interventions for Care Transitions? Download the Cross Setting III BPIP and read page 26. This includes a description of the Seven Essential Interventions (National Transitons of Care Coalition-NTOCC) and a link to the NTOCC Care Transitions Bundle.
