 Cross Settings I BPIP

A new Home Health Quality Improvement (HHQI) National Campaign Best Practice Intervention Package (BPIP) debuted in October and is available for you to download now. This BPIP focuses on improving care transitions and aligning with other health care providers to work together to decrease avoidable hospitalizations. The Cross Settings I BPIP is full of tools, links and information from experts who specialize in care transitions. 

 

The home health setting is in a great position to contribute to patients making a safer transition across settings. Make sure you take advantage of the great resources in this package by downloading the Cross Settings I BPIP today. In it you'll find a Focus on Care Transitions section and authentic Success Stories featuring Seton Home Health, Amedisys Home Health Services, Eddy Visiting Nurse Association/Northeast Health and Gentiva of Nashville. They offer useful tips to help you improve care transitions or even implement a coaching program at your agency.

Remember that in mid-2011 every HHQI National Campaign participant who downloads the final four campaign BPIPs will be provided a special electronic gold metal insignia denoting their special status as an engaged participant. It's simple: 

 

1. Log in to the HHQI site with your username and password here

2. Click on "My Links" in the gold toolbar
