The Home Health Quality Improvement (HHQI) National Campaign recently hosted a free Webinar called Making Care Transitions a Reality Through Home Health/Implementing the Coaching Model. Tasha Mears, Senior Vice President of Clinical Operations from Amedisys and Beverly Taylor, Care Transitions Coordinator from Amedisys Georgia presented on the Webinar about what worked for them in implementing the coaching model at their agencies.
 
Implementing the Coaching Model netted more than 1,000 live participants on November 12. The Webinar is currently archived for viewing here. A copy of the slides is also available on the HHQI Web site. Make sure to view this 60-minute Webinar broadcast as soon as possible, since this valuable information for all home care providers and stakeholders.

The Webinar, Conversations Across the Discharge Divide: Evidence-based Communication Among Patients, Hospital-based Providers, and Community Healthcare Resources, will be presented by Northwestern University faculty and staff.

Optimizing Home Health in Care Transitions Summit

HHQI National Campaign Medical Director David Wenner, DO, FAAFP recently delivered a keynote address on the HHQI National Campaign and Quality Improvement Organization (QIO) Care Transitions program at the inaugural Optimizing Home Health in Care Transitions Summit in Philadelphia. The summit was sponsored by The Joint Commission, the University of Pennsylvania School of Nursing and the Alliance for Home Health Quality and Innovation.

Agencies Working Hard on Falls Prevention
 

Recent HHQI evaluation shows that agencies are working hard on falls prevention. Three months after the release of a related Home Health Quality Improvement (HHQI) National Campaign Best Practice Improvement Package (BPIP), home health agencies have reported wide adoption of methods to protect patients through better falls prevention. Have you downloaded the Cross Settings I Best Practice Intervention Package yet? The Cross Settings I BPIP features several interdisciplinary tools and a Focus on Care Transitions and Coaching section that compiles tips and advice from experts in the field.

The goal of this package is to provide home care leaders with guidance for selecting best practices for improving care transitions and aligning with other health care providers. The ultimate goal is to work together to decrease avoidable hospitalizations and improving quality of care. The BPIP will also introduce clinicians with best practice tools and resources to assist them with improving care transitions for their patients. Share this BPIP with friends in other settings.
