Cross Settings I BPIP Released
We are very excited to announce the release of the Cross Settings I Best Practice Intervention Package (BPIP). The Cross Settings I BPIP features several interdisciplinary tools and a Focus on Care Transitions and Coaching section that compiles tips and advice from experts in the field.
The goal of this package is to provide home care leaders with guidance for selecting best practices for improving care transitions and aligning with other health care providers to work together to decrease avoidable hospitalizations and improving the quality of care. The BPIP will also introduce clinicians with best practice tools and resources to assist them with improving care transitions for their patients. Share this BPIP with friends in other settings! HHQI National Campaign October Agency of the Month
Community Hospital Home Health
"Community Hospital Home Health has a team of dedicated staff that promotes quality of care. The staff is all involved in monitoring and assessing the care provided by reviewing indicators that measure the quality they work so hard to deliver. One change our agency has made to help promote quality of care was to develop a Unit Practice Council where all of the staff meets to discuss quality of care, patient satisfaction, and other processes. The staff meets regularly to review indicators such as acute care hospitalization and oral medication management, and together they develop action plans that will facilitate even better quality of care. This team communication and empowerment to make positive changes has been a key factor in improving the quality of care our patients receive. We are fortunate to have such a great team of professionals that work very closely together to assure that our patients receive the best care!"

HHQI Free Webinars

The Home Health Quality Improvement (HHQI) National Campaign recently presented nationally recognized transitional care expert Dr. Jane Brock during a free Webinar called Home Health and Care Transitions. Dr. Brock is Chief Medical Officer of the Colorado Foundation for Medical Care, the Quality Improvement Organization Support Center (QIOSC) leading 14-QIOs across the nation in Care Transitions.
Home Health and Care Transitions netted more than 1,000 live participants on September 29. The Webinar is currently archived for viewing at www.homehealthquality.org. A copy of Dr. Brock's slides is also available on the HHQI Web site. Make sure to view this 60-minute Webinar broadcast as soon as possible, since this valuable information for all home care providers and stakeholders. Achieving the Joint Commission's "Gold Seal of Approval" can distinguish home health agencies as a quality-focused partner to referral sources, key stakeholders and others within their service area.
The Joint Commission's home care program and survey processes are uniquely patient centered and focus on the quality of care provided. Their standards address critical areas that can most impact patient care and safety such as hand-off communications, medication management, and the reduction of falls. Additionally, the Joint Commission's on-site survey process evaluates a patient's care experience in real time and under normal business conditions by using an interactive "tracer" process. All components of the Joint Commission's home care accreditation program provide unparalleled learning and improvement opportunities for home health agencies' staff and leadership teams.
