Falls Prevention Awareness
Falls Prevention Awareness Week is being observed from September 20-26 all over the nation. Thirty-five states will also observe Falls Prevention Awareness Day on September 23, the first day of fall, according to the Center for Healthy Aging. Learn more by clicking here. The Association for Home & Hospice Care of North Carolina, which serves as the North Carolina HHQI Local Area Network for Excellence (LANE), has graciously provided the HHQI National Campaign with many of the resources it's using for Falls Prevention Week. Modify them or use them to glean ideas for hosting activities in your state. You can access them here. The Joint Commission also recently launched a national campaign to help Americans reduce the risk of falling. The new education campaign, which is part of their award-winning Speak Up™ program, recognizes that falls are a serious problem. You can download program materials and order free easy-to-read fall prevention brochures in English or Spanish. Learn all about the Speak Up program here. In addition, the HHQI National Campaign Fall Prevention Best Practice Intervention Package (BPIP) features a Focus on Falls Section (pages 13-20) and Success Stories. Read about how Holyoke Visiting Nurse Association and Brockton Visiting Nurse Associations implemented a fall prevention program. Home Health and Care Transitions
Registration is closed for the September 29 live Home Health and Care Transitions Webinar. However, you can bookmark this link and check back October 1, 2010, for an archived version of the Webinar, which features Jane Brock, MD, MSPH, Chief Medical Officer of Colorado Foundation for Medical Care. Dr. Brock will provide an overview of CMS's Care Transitions Theme as well as an update on the latest ideas about hospital readmissions and coordinating care. She will also discuss the early results of efforts to reduce hospital readmissions through community-based transitional care improvement, and she will describe the role of home health in the Care Transitions Communities. The Colorado Foundation for Medical Care (CFMC) is the Quality Improvement Organization Support Center (QIOSC). CFMC leads 14 QIOs across the nation in a Care Transitions project to improve coordination across the continuum of care. Click here to learn more.
Sneak Preview: The Coaching Model
The HHQI National Campaign will present a free nationwide Webinar on the coaching model in home health care transitions in November. The 60-minute episode will include practical applications that show how home health agencies are effectively implementing the coaching model. Hospital Readmissions: The Role of Home Care
The HHQI National Campaign offers an archived Webinar of nationally renowned home care expert Dr. Steven Landers' presentation, Hospital Readmissions: The Role of Home Care. Dr. Landers, who was selected as the 2009 Physician of the Year by the National Association for Home Care and Hospice, directs the Cleveland Clinic's Center for Home Care and Community Rehabilitation. The 60-minute interactive session, which was delivered live in June 2010, is geared to reducing unnecessary hospital 30-day readmissions. It includes best practices for the use of home health care with high-risk patients. It discusses the benefits of home health care referrals for patients in post-acute care transition and also offers tips for better cross-setting communication in the hospital discharge process.
