HHQI Data Access
Enhancements | |
The HHQI National Campaign has upgraded its data reporting capabilities on the HHQI Data Access site. Several new viewing and download options have been added for your convenience. Along with the existing PDF file format and CSV download capability you will now be able to select XML, MHTML, Excel, TIFF and Word as alternative viewing and download options. Simply select the report to view and scroll down to the method that meets your need from the 'Select a format' drop down. The report will be downloaded/displayed in the format that you have selected. Historical data reports are now included on the site so you can view your progress over time. We have also added state and national rates to each report so you can see how your agency stacks up against national rates. Please note -- this data is only available beginning with the July 2010 data report. Historical data reports have not been modified and will not have the comparative numbers. As with everything in the HHQI National Campaign, customized Data Access reports are absolutely free. If you're not a Data Access participant already, click here to sign up today! | |

HHQI Rewards Launches Today
The Home Health Quality Improvement National Campaign team is excited to announce a new HHQI Rewards incentive program for all participants. HHQI Rewards consists of two initiatives designed to offer visibility incentives for being an active and engaged participant in the campaign. They are called HHQI Agency of the Month & Go for the Gold!
HHQI Agency of the Month The HHQI Agency of the Month (AOTM) contest begins today and will run through June 2011. To be eligible, you simply fill out a form by clicking here. The form asks several simple questions about your participation in the campaign. Each field must be completed, and participants need to be in the 90th percentile or better of combined ACH & Medication Management rates to be eligible. Starting in August, the HHQI team will randomly select an Agency of the Month (AOTM) from all completed entries submitted by the 25th of each month. The winning participant will be notified by e-mail. As AOTM, the agency will be honored with a prominent listing on the front page of the HHQI Web site and in future e-Bulletins. Submit your entry now to be Agency of the Month by clicking here. You can enter every month, so make sure to bookmark the page and visit often. And don't forget to check the HHQI National Campaign Web site in early September for our first HHQI AOTM. It could be you! Go for the Gold! Go for the Gold! is a HHQI Rewards incentive program offering special recognition for participants based on successful download of the final four HHQI Best Practice Intervention Packages (BPIPs) in the 2010-2011 HHQI National Campaign. Participants who download all four of the final BPIPs will be provided a special end-of-campaign electronic gold metal denoting their special status as an engaged participant. Other participants will receive silver or bronze insignias depending on number of downloads tracked. But the goal is for everyone to Go for the Gold! Starting in a few weeks, the HHQI team will begin reminding participants via email who haven't yet downloaded the latest BPIP. A complete list of BPIP release dates is available by clicking here. The deadline for downloading will be June 1, 2011 (one month following the release of the final BPIP). But you can get started by downloading the latest BPIP about Fall Prevention right now. Free Hospital Readmissions Webinar
The Home Health Quality Improvement (HHQI) National Campaign recently presentednationally renowned home care expert Dr. Steven Landers during a free Webinar entitled Hospital Readmissions: The Role of Home Care.
The American Hospital Association (AHA) helped promote the June 30 event, which netted more than live 1,500 participants, including many hospital leaders. Dr. Landers, who was selected as the 2009 Physician of the Year by the National Association for Home Care and Hospice, directs the Cleveland Clinic's Center for Home Care and Community Rehabilitation.
You can view the Webinar and Dr. Lander's presentation slides by clicking here. The 60-minute interactive session, which is geared to reducing unnecessary hospital 30-day readmissions, includes best practices for the use of home health care with high-risk patients. Dr. Landers explains the benefits of home health care referrals for patients in post-acute care transition and also offers tips for better cross-setting communication in the hospital discharge process. A lively Q&A session follows.
The event is the first in a series of free educational Webinars sponsored by the HHQI National Campaign. The next session will take place September 29 featuring Dr. Jane Brock, Chief Medical Officer of the Colorado Foundation for Medical Care. Dr. Brock will present a care transitions update for the nation's home care community and various cross-setting stakeholders.
Registration begins soon at www.homehealthquality.org, so visit the HHQI National Campaign Web site often to reserve your spot for our next Webinar event. |
From Florida to Nebraska, the Home Health Quality Improvement Campaign team has continued to spread the word this summer about ways to improve care.
Project Director Shanen Wright and RN Project Coordinators Eve Esslinger and Cindy Sun have extended an open offer to present at conferences and symposiums to their base of 120+ Local Area Networks for Excellence (LANEs). LANEs are grassroots organizations that serve as campaign communicators and motivators for participants in their state or region. In order to accommodate the high volume of requests and ensure equity, HHQI staff require travel reimbursement to present at the conferences.
"This is a great way for us to interact face-to-face with our enormous base of participants and supporters from all over the nation," Wright said. "These presentations are also excellent opportunities to recruit new partners and participants at these well-attended events."
In its first six months, the HHQI National Campaign has amassed more than 7,000 participants and supporters from every state and territory in the nation. "This is a great number, but we're all about quality, not quantity," Wright said. "Fortunately our initial survey results are showing a high level of participant engagement and satisfaction with our tools, so we're getting the best of both worlds." To request a member of the HHQI team to present at your next event, contact us at hhqi@wvmi.org.
