The April Best Practice Intervention Package (BPIP) - Improving Management of Oral Medications - is now available. To access the above link, make sure you are logged into the site. In this BPIP you will find a wealth of information, tools and resources to help assess and improve patients' ability to manage medications. You can download the "Full Contents" or download specific tracks related to your discipline. Be sure to check out the success stories from Dominican Sisters Family Service and Lakeland Home Care for tips and tools from these agencies that lead to their success in managing medications.

Acute Care Hospitalization Monthly Report Inclusion Criteria
- Patients discharged from an Agency and the episode of care did not end with a death at home
- Patients aged between 18 and 120 years of age
- Patients who have not reported a value of NA (Patient nonresponsive) for M0570 (When Confused)
- Patients who have not reported a value of NA (Patient nonresponsive) for M0580 (When Anxious)
Note: Reports are not produced for agencies with yearly discharges of less than 20 Improvement in Oral Medications Report Inclusion Criteria
- Patients discharged from an Agency
- Patients aged between 18 and 120 years of age
Note: Reports are not produced for agencies with less than 2 discharges Reports for OASIS-C M2000 (Drug Regimen Review), M2002 (Medication Follow-up), M2004 (Medication Intervention), M2010 (Patient/Caregiver High Risk Drug Education), M2015 (Patient/Caregiver Drug Education Intervention), age and race groups are evaluated based on improvement in management of oral medications (M2020) The display titled 'Oral Medication Improvement Rate (%) by Medication Follow-up (M2002) Groups', the value for NA is derived from those instances where M2000 (Drug Regimen Review) is equal to a 0 (Not assessed/reviewed) or a 1 (No problems found during review)
