Campaign Participation Update

The second phase of the Home Health Quality Improvement (HHQI) National Campaign is off to a great start. As of 2/16/2010 we have 4,898 registered users from 3,729 home health agencies. Thank you for joining the movement to evolve health care through quality improvement: the HHQI National Campaign.

BPIP Release

Registered campaign participants receive exclusive access to Best practice Intervention Packages (BPIPs), which can help focus and drive quality improvement efforts.

The first Best Practice Intervention Package (BPIP) of the new campaign was released on January 28, 2010. The introductory package, Fundamentals of Reducing Avoidable Acute Care Hospitalizations, has several areas that may be of interest, including:

Introduction: The 6 page introduction gives participants an overview of the campaign, comparison with the 2007 campaign, and introduces Care Transitions. The introduction section will help participants understand the purpose and objectives of the campaign.

Leadership: Agency leaders should carefully review the leadership section. The leadership section gives a suggested timeline and agency checklist for using the BPIP. The central feature of the Leadership section is the Checklist for Agency Leadership. This document gives suggested activities for agency leaders and implementation teams and is designed to offer flexibility to the many home health agencies that participate in the campaign.

Discipline Tracks: The discipline tracks are concise handouts for specific staff, nurses, therapists, medical social workers and home health aides. A specific discipline checklist is included as well as INSIGHTS, application principles from other home care clinicians.

To download the BPIP, Click the LOG IN logo on the front page of www.homehealthquality.org. Next click on MY LINKs on the top navigation bar, then click Introduction BPIP. Please be aware that once the BPIP is downloaded, the links within the BPIP will only work if you are still "LOGGED IN."

The next BPIP focusing on Oral Medication Management will debut on April 28. To view a listing of all BPIP releases, click here

HHQI Summit Recap

The new 2010-2011 Home Health Quality Improvement National Campaign kicked off on January 13, 2010. Over 300 people representing Quality Improvement Organizations (QIOs), health care industry leaders, home health agencies, and CMS attended the kick-off summit. Speakers at the event included Dr. Steven Landers, director of the Cleveland Clinic's Center for Home Care and Community Rehabilitation, Dr. David Wenner, medical director for Quality Insights of Pennsylvania, the Medicare QIO for Pennsylvania; and various members of CMS leadership. To view presentations from the summit, click here.  Also available are the two videos, The Home Health Experience and Perspectives on Home Health, which premiered at the summit and show viewers an inside look at home health. The videos are also available on YouTube.

Priority Populations

The HHQI National Campaign includes a significant focus on racial, ethnic and geographic priority populations. Our efforts are currently focused on an in-depth analysis of disparities in acute care hospitalization and medication management quality to see whether race-specific or geographic differences are significant within or between home health agencies. Where we find such differences at the agency level, we will develop resources aimed at helping staff provide equal and high quality, culturally sensitive care.

In addition to conducting this in-depth data analysis, we are also continuously developing home health industry and agency partnerships and continuing to engage participants through innovative communications strategies related to disparities. Please contact us to learn more and join our efforts to reduce disparities and improve the quality of home health care.
