In order to proceed with this program, we need to confirm the numbers. GoodLife requires at least 100 OAVT participants to sign up in order to move forward with the preferred rate.
Membership with GoodLife can commence on June 1st, however, in order
to do so we will need to confirm participation by May 20th, 2011.
Please fill out the registration form, along with payment info, prior to Friday, May 20th.
If enough OAVT members respond, then your payment will be processed prior to June 1st and your membership card will be sent to your home address.
The rate will be $350.00 + HST for residents of Ontario. This preferred rate can be used by all family members in your household.
If you currently have a GoodLife membership your new OAVT corporate membership with the discounted rate will begin July 1st, 2011. This will allow adequate time for your current membership to be cancelled.
If the OAVT does not receive confirmation by May 20th from at least 100 participants, you will not be billed, and this program will not proceed.
A Cancellation Letter will be emailed to you to bring into your local club (if you have biweekly payments that need to be stopped).
