CMPW Late Equipment handout!
Handout Schedule:
 
Flag, JMM, MM, JPW & PW-8:30 - 9:30am (first come first serve)
 
JM & Midget......9:00 - 11:00am (first come first serve)
Be sure to have the following items (if you have not already provided them):
    1.   Registration PAID IN FULL
    2.   Completed, Signed Contract - Will pass out if still need.
    3.   Copy of Birth Certificate
    4.   Original Birth Certificate (to show)
    5.   Utility Bill - or other address verification
    6.   Small Photo of player
    7.   3 copies of final report card.
    8.   LETTER FUNDERAISER!! - in manila envelope with team and individual name.

Important tid-bits to note:
$25 late fee for late equipment handout, unless you have been previously notified by your team mom. 

Parking:Fairview Park off Placentia Ave OR Estancia High School.

No equipment will be handed out without full payment of registration.
