Don't forget the following documents: 
 
1 copy of a Birth Certificate and the original (verification purposes) which you will keep
1 utility bill-current (address verification)
1 Physical/Medical Form (if not completed yet you can do so that day)
1 2x2 current photo of player
3 Copies of report cards (to be collected at a later date)
