"RULES FOR SUBMISSION" OF NOMINEE
13th ANNUAL 2011 SENIOR ACHIEVEMENT AWARDS
Please print or type nomination application as completely as possible. If necessary, you may attach up to a maximum of two additional pages. Nomination to the "2011 Senior Achievement Award" must be completed and returned by 5:00 p.m. on Monday, October 3rd 2011.

The 13th Annual "Senior Achievement" luncheon will be held on Friday, November 11, 2011 at the Benvenuto Restaurant in Boynton Beach., Florida. All recipients are required to attend the luncheon and each recipient will be the guest of the American Health Foundation and the Senior Achievement Project.
RULES FOR SUBMISSION
- All nominees, volunteers or paid staff must be a member of a not-for-profit
- 501 (C)-3 charitable organization chartered in the United States by the Internal Revenue Service and located within Florida as a home base or branch.
- All nominees must have demonstrated service beyond the call of normal duty contributing to both the ideals of their respectful agency or organization, the spirit of community service and dedication to a particular program, project or event of said charity.
- The program, project or event associated with the nominee should reflect the ideals, vision and mission of the nominating agency or organization.
- All nominations submitted will be judged for their contribution to the community and the spirit of community service. The decision of the AHA Board of Trustees and the Senior Achievement Advisory Panel are final. The awards will be made at the 13th Annual "Senior Achievement"
- Awards Luncheon held this year on Friday, November 11th 2011.
- Please use only official Nomination Application Form to submit your nominees.
- Official Forms are available at the offices of American Health Association, 1181 South Rogers Circle, Suite 2, Boca Raton, FL 33487. No other form should be used or print the form using this link.
- Senior Nomination Form
- Winning recipients will attend the Senior Achievement Awards Luncheon as guest of the Foundation. All other guests will be expected to pay the normal luncheon rates per the invitation and menu.
- The Senior Achievement name and the use thereof, are exclusive trademarks of the Senior Foundation and the American Health Foundation. It cannot be used without the expressed written consent of the Board of Trustees of the American Health Association.
- All Senior Achievement nominations will be judged by an impartial jury and the Judge's decisions are final. A list of the award recipients will be available on or after October 15th 2011 by writing:
