Ask The Doctor
Although I am only 45 years old, I am seeing deep wrinkles around my mouth with thickened skin. What can I do? Will fillers help???
I hear this complaint often and have noticed that most patients with this problem, also have a parent with the same issues. There seems to be a genetic component to aging that is unavoidable but definitely correctable. Certain facial expressions such as unconscious puckering of the lip and smoking seem to accelerate this condition.
The most effective treatment for deep wrinkles above and/or below the lip is perioral dermabrasion. The dermabrasion procedure should be virtually painless. During the
procedure, a burr with diamond particles attached to
a motorized handle is utilized to scrape away the top layer of skin. The skin is abraded to a level that is safe, making sure that the
wrinkle or thickened area is dramatically softened.
If the upper lip wrinkles are not yet at a point that dermabraion would be of benefit but are still visible, I often outline the border of the lip with a hyaluronic filler such as Prevelle Silk or Restylane. The lip does not look appreciably larger yet it has more definition and the small lines are much less noticeable.

Less Frequent Doses Of Botox Still Result In Reduced Wrinkles!

| Patients can decrease the frequency of Botox©
Cosmetic injections after approximately two years and still receive
most of the same wrinkle-smoothing cosmetic benefits, according to new
research at Oregon Health & Science University.
"After two years of treatment at recommended intervals,
patients can potentially cut the frequency, and thus the cost, of their
Botox© treatments by half," said Roger A. Dailey, M.D., F.A.C.S.,
professor and Lester Jones Endowed Chair of oculofacial plastic surgery
in the OHSU School of Medicine. The results of Dailey's work were
presented at a meeting of American Society of Aesthetic Plastic Surgeon
on April 24 in Washington, D.C. The research was sponsored by an
unrestricted educational grant from Allergen, Inc., the maker of Botox©
Cosmetic.
The Botox© research effort also demonstrated that the
injections have a wrinkle preventing - or prophylactic - effect.
Patients who begin receiving injections between their 30s and 50s are
able to prevent wrinkles from forming and eliminate existing wrinkles,
said Dailey.
Based on previous studies, doctors advised patients who wished
to reduce wrinkles in the glabellar region - the area between the
eyebrows - that they needed to have Botox© Cosmetic injections every
three months to maintain the cosmetic wrinkle-smoothing benefits. Such
frequent treatment, however, deterred some patients, Dailey said.
Dailey studied 50 women ages 30 to 50, who received regular
Botox© injections for two years. "We found that after the patient
receives Botox© Cosmetic injections every four months for two years,
the frequency of the injections can be changed to every six months and
still achieve good results," Dailey said. "This demonstrates patients
have the ability to achieve good results with broader treatment
schedules and ultimately at a lower overall treatment cost.

Happy Mother's Day! May brings our beautiful Marin County weather and along with it the temptation to enjoy the multiple outdoor activities that are available in sunny California. I would like to remind you of the importance of my two favorite safety choices, sunscreens and bicycle helmets.
Last month brought several interesting studies that were presented at national meetings. Later in this newsletter, I have reported on a study that was done on patients between 30 and 50 years of age. It demonstrated that after Botox was injected at 4 month intervals for 2 years, consequent Botox injections could be decreased to every 6 months to maintain less visible wrinkling. I would attribute this phenomenon to the decrease in repetitive wrinkle creasing because the muscles are weakened. Constant pleating of the skin is reduced as is habitual facial tendency to create the expression. That being said, if no movement of the treated areas is your goal, you will probably want to maintain the schedule of injections that currently is effective for you. Natural facial expression is an aesthetically pleasing component of a face. To weaken and soften deep wrinkles is the goal, not to eliminate all expression lines completely.
The second study analyzed the facial changes that occur in the angle of the jaw as we age. The possibility of adding facial implants to skin reduction and muscle tightening procedures is discussed. One area that I commonly see the need for a facial implant is in the area of the chin. I find that defatting and resuspending the neck and redefining the chin with an implant for many patients makes a huge difference.

Basically, a "mommy makeover" is any combination of breast and body procedures that may address abdominal laxity (fat,skin and/or muscle), breasts that have been altered by pregnancy or breast feeding and fat deposits that have occurred elsewhere after childbearing. Of course, every woman will have different concerns and issues so the "mommy makeover" is tailored to whatever correction is desired individually. I have also included a gallery of some of my patients that have elected to undergo various versions of this procedure.

Lastly, a fun study where plastic surgeons were polled about the aesthetically ideal faces of the past few decades. When I looked at their choices, I was reminded of the "golden ratio" that I learned in training. Classic beauty is most often seen in a face that can be equally divided into thirds lengthwise (front hairline to middle of eyebrows, mid brows to tip of
nose, and tip of nose to tip of chin). If these are equal your face
considered harmonious. Of course, beauty is always unique and few of us, including me, can claim to be golden!

Featured Procedure: Mommy Makeover
|  Life is full of gifts and although the addition of children to a
family is wonderfully anticipated, the resulting changes to the
mother's body often are not. Today's mothers agree that their families
are their first priority but are also clear that the ability to
recontour the body to the pre-pregnancy appearance is easier than ever
before to achieve.
Breast Changes and Correction
The
physical changes that can be addressed with Mommy Makeover breast
rejuvenation are drooping breasts, large, sagging breasts or breasts
that have simply lost volume. During pregnancy and lactation, the
breast first prepares for breast feeding by the increase in size of the
milk glands. The remaining fat in the breast shrinks and the skin
capsule enlarges at the same time. When lactation is complete, the milk
glands return to their normal size but the skin of the breast may be
stretched and pre-pregnancy fat in the breasts is diminished. The woman
is left with breasts that are less full and commonly have lost volume
in the top of the breast, tend to sag lower than before the pregnancy
and often will have developed stretch marks. Most stretch marks may
fade out over time but the loose skin will contract minimally.
The
procedures most often used in Mommy Makeover breast rejuvenation
include breast augmentation alone, breast augmentation and breast lift
together or breast reduction. During consultation, Dr. Hand will make
individualized recommendations based on the loss of volume of the
breast, amount of sag of the surrounding skin and the preference for
eventual breast size. Sometimes when only a small amount of breast sag
has resulted from pregnancy, breast augmentation as a corrective
procedure is enough. Most often when a large amount of sag is present,
a breast lift is indicated and a breast augmentation may be added if a
fuller breast with more volume is desired. If the breast remains
excessively large after pregnancy and lactation, a breast reduction may
be indicated to remove part of the breast tissue and restore the breast
to a volume that fits the body and has a more youthful appearance. Dr.
Hand recommends that these procedures be done after the completion of
one's family and 3 months or later after the completion of
breastfeeding. Future pregnancies will recreate the hormonal influences
that caused the stretching in the past. Newer techniques in breast
surgery usually preserve the ability to breastfeed afterwards but
cannot be guaranteed, particularly with breast reduction.
Abdominal Changes and Correction
Perhaps
the most significant bodily change of pregnancy is seen in the
abdominal muscles and skin. As a pregnancy progresses, the mother's
hormones allow the abdominal muscles to relax. Small hernias are not
uncommon, occurring because of the resulting separation of the muscles
as the baby's size continues to increase. Sometimes, particularly in a
youthful mother, the abdomen will return to its pre-pregnancy state but
most often varying amounts of loose, possibly sagging skin occur. The
woman may be left with stretch marks, particularly in the lower
abdominal area and bulging muscles.
The procedures
used for Mommy Makeover abdominal rejuvenation include tummy tuck
(abdominoplasty), mini-tummy tuck and liposuction. The abdominoplasty
procedure is performed in our accredited Marin County outpatient
surgical facility. Dr. Hand commonly advises a regular tummy tuck for
most women to remove the large pooch of skin and fat below the belly
button to the upper pubic area, to tighten muscles and to remove
underlying fat in the lower abdomen. The belly button is repositioned,
lower abdominal stretch marks are removed, internal muscles of the
upper and lower abdomen are tightened and the upper and lower abdomen
regain the tight, toned appearance lost after pregnancy. The surgical
incision is usually curved, extending from hip to hip under the bikini
line above the pubis so that it will be hidden in a bikini. The scar is
usually a fine line that continues to fade. When a woman has only a
small amount of lower abdominal laxity, a procedure called a mini-tummy
tuck may be utilized. It is a more abbreviated procedure in that only
the lower abdominal muscles may be tightened slightly and the belly
button is not repositioned. When a tummy tuck is not an option for a
woman and extra skin is not an issue, liposuction is sometimes
performed to remove fatty deposits to the abdomen, flanks, lower
extremities and arms.
Abdominoplasty while being a
wonderful solution to the issues that have been previously addressed,
is a procedure that requires the woman has adequate help at home to
deal with small children. Dr. Hand advises no lifting over 10 pounds
for 2 weeks. Regular intense exercise may not be resumed for 4-6 weeks.
Most women expect to need help with carpooling and daily activities for
the first 1 to 2 weeks.
Although abdominoplasty
can be performed earlier, Dr. Hand recommends that the procedure be
performed 6 months or longer after delivery in order to allow the skin
to maximally contract naturally.
Localized Fat Deposits and Correction
Very
often, mothers find that their bodies continue to change for up to a
year after pregnancy, but localized fat deposits tend to stubbornly
stay. Liposuction is now performed in most areas of the lower
extremities, abdomen, back, arms and neck. It is best used for the
woman that at a consistent weight with localized fat deposits that are
resistant to diet and exercise.
Dr. Hand can also
perform most of the above procedures at the same time as tubal ligation
or hysterectomy, in conjunction with the patient's obstetrician at the
hospital. Multiple procedures can sometimes be combined and often are
staged at our accredited Marin County outpatient surgical facility into
2 surgical days to allow for easier recuperation.
Other Options
Although
in most cases Mommy Makeover involves body rejuvenation, other options
to refresh the facial appearance are also available. Upper or lower
eyelid surgery can revive the tired look many new moms justifiably
experience. Mini-facelift and dermal fillers can freshen the beginnings

Facial Aging Is More Than Skin Deep
|

Facelifts and other
wrinkle-reducing procedures have long been sought by people wanting to
ward off the signs of aging, but new research suggests that it takes
more than tightening loose skin to restore a youthful look. A study by
physicians at the University of Rochester Medical Center indicates that
significant changes in facial bones - particularly the jaw bone - occur
as people age and contribute to an aging appearance.
Presented today at the American Association of Plastic Surgeons annual meeting in San Antonio, Texas, and published in the Journal of Plastic and Reconstructive Surgery,
the study suggests that the future approach to facial rejuvenation may
be two-fold, first restoring structure underneath before performing
skin-tightening procedures.
Reviewing a collection of 120 facial CT scans taken for other,
unrelated medical reasons, plastic surgeons measured changes that
occurred to facial bones over time. The CT scans were divided equally
by gender and age, 20 men and 20 women in each of three age groups:
young (ages 20-36), middle (41 to 64), and old (65 and older).
Researchers used a computer program to measure the length, width, and
angle of the mandible, or jaw bone, for each scan, and compare the
results for each group. Using CT scans for this study allowed for more
accurate three-dimensional reconstruction and increased accuracy of
measurements, disputing previous research that relied on traditional
head x-rays and suggested that the jaw bone expands with age.
The angle of the jaw increases markedly with age, which results in
a loss of definition of the lower border of the face, according to the
study. Jaw length decreases significantly in comparisons between the
young and middle age groups, whereas the decline in jaw height from the
middle to old group was noteworthy.
"The jaw is the foundation of the lower face, and changes to it
affect facial aesthetics," said Howard N. Langstein, M.D., professor
and chief of Plastic and Reconstructive Surgery at the University of
Rochester Medical Center. "These measurements indicate a significant
decline in the jaw's volume as a person ages, and therefore less
support of soft tissue of the lower face and neck."
This loss of bony volume may contribute sagging facial skin,
decreased chin projection, and loss of jaw-line definition. As jaw
volume decreases, soft tissue of the lower face has less support,
resulting in a softer, oval appearance to the lower face and sagging
skin, which also affects the aging appearance of the neck.
"Physicians have long been taught that facial aging is caused by
soft tissue descent and loss of elasticity," Langstein said. "Though we
have always known that bones change over time, until now, the extent to
which it causes an aged appearance was not appreciated."
The study by Dr. Langstein and Dr. Shaw, gives evidence that facial bones are constantly subjected to
forces that remodel them. Understanding there are predictable changes
in facial bone structure as people age gives physicians new insight
into procedures that may successfully restore youthful appearance. Shaw
and Langstein led the three-institution collaboration, which also
involved Stanford University and Harvard University.
"The future of facial cosmetic procedures to restore a youthful
look may include methods to suspend soft tissue - such as chin and
cheek implants - to rebuild the structure that time has worn away, in
addition to lifting and reducing excess skin," Shaw said.

Plastic Surgeons Reveal The Top Celebrity Faces Through The Decades
| The following information highlights the celebrity beauty findings from the recent
AAFPRS membership survey:
The 1960's  
Peace, love, happiness and wide, dramatic eyes marked the sixties. Legendary actors Elizabeth
Taylor (38 percent) and Paul Newman (42 percent) defined beauty during this decade. Taylor's porcelain skin and well-proportioned eyes as well as the defined features
of Newman's eyes and lips epitomized timeliness beauty. Fair Lady Audrey
 Hepburn
followed Taylor with 26 percent of the votes, while the tall, dark and handsome
Cary Grant rounded out the male category with 38 percent.
Plastic surgeon experts agree that facial skeletal proportions are the
true determining factors when analyzing facial attractiveness. A well-proportioned
face should be divided into equal thirds, with horizontal lines drawn through the
anterior hairline, the brow, the base of the nose, and the edge of the chin.
The 1970's Disco, bellbottoms, polyester and thick eyebrows characterized the seventies
with icons Farrah Fawcett and Robert Redford both receiving 70 percent of the facial
plastic surgeon votes. "The seventies embodied leading ladies who had defined features
like Meryl Streep and Cher," said Pearlman. "But Farrah Fawcett's delicate nose
and chin and well-developed cheek bones gave her the face of this decade, while
Robert Redford's nose and strong jaw line illustrated why he was selected."
The 1980's
 Eighties glitz; big hair and bold makeup represented the beauty ideal of this
time. Model Christie Brinkley (49 percent) and actor Mel Gibson (33 percent) embodied
the best facial features during this decade while Michelle Pfeiffer (38%) and Harrison
Ford (30%) come in a close second.
The 1990's  Minimalism, grunge and supermodels indicated a change in the ideal
face of beauty. Fashion icon Cindy Crawford (40 percent) and legendary bachelor
George Clooney (29 percent) symbolized attractiveness in the nineties. "Supermodel
Cindy Crawford was likely chosen by our members because of her flawless skin, attractive
nose and recognizable lips. Clooney possesses a strong jaw and straight, masculine
nose," said Pearlman. Starlet Nicole Kidman came in second with 14 percent of the
votes as did Tom Cruise with 22 percent.
The Present
 Catherine Zeta-Jones (31%) and Hugh Jackman (26%) epitomize the current ideal
faces of beauty. Charlize Theron (23%) comes in second and Lucy Liu (1%) gets only
a small vote. In the male category, Colin Farrell (21%) is right behind Jackman
while Justin Timberlake (3%) comes in last. The present faces of beauty represent
many features of the ideal.
Although it is fun and interesting to review the standards of beauty over the decades, it is important for
prospective patients to have realistic expectations of facial plastic surgery and
should seek cosmetic surgery only to enhance features rather than to look like someone
else. |
