What is your favorite surgical procedure to perform and why??? H.P.
My favorite surgical procedure to perform has to be upper and/or lower eyelid lift (blepharoplasty) because each set of eyelids has different artistic challenges. In my practice, I find that nothing changes the appearance of a face that is starting to age as much as correcting drooping eyelid skin, excess fat deposits and lax muscles. The added boost of lifting the midface upward and repositioning the cheek fat to it's youthful position makes a huge difference in facial balance. The face appears refreshed and most patients are delighted to regain their gorgeous eyes.
Only because I had a tough time choosing my favorite procedure, I must say that liposuction is a close second. Vaser ultrasonic liposuction provides wonderful results with minimal pain and down time. My most common areas of treatment are abdomen and flanks although almost any area from the knees to the neck can be treated with wonderful results. |

Since summer is a time for all of us to relax and remember why we live in this beautiful place, I am practicing what I advocate and combining the July and August issues of my newsletter. Call me a slacker but I hope all of you are enjoying your summer adventures as well!
This month's educational tool is from the Latisse website and features their spokeswoman Brooke Shields. Although I can honestly say I have not kept up with her acting career, I have watched the progress of her eyelashes during the 12 weeks she was photographed! Her experience with Latisse seems to mirror most of my patients in her video diary. Lisa and Cheri are now maintaining their eyelash growth with an application of Latisse 2-3 times per week as are most of our other patients that have completed the initial 16 week treatment course.. I can honestly say that this is a product that really delivers the longer and fuller eyelashes that it promises and has been greatly enjoyed by a range of patients.
This months newsletter discusses 2 of my pet peeves, cosmetic surgery "vacations" and the ever annoying trend to rename and represent old procedures as new, life changing surgical options that promise no down time with amazing results. Although I do believe that there are many procedures including fillers, Botox and some lasers that make significant changes quickly and easily, surgical options always require preparation and planning for 1 to 2 weeks of recovery. As I tell all who ask, sometimes surgery is the best and only solution. Vacations are best done prior to surgery since an increased activity level and your post surgical appearance make most vacations an impossibility after the completion of most surgical procedures.
This month's featured procedure is peri-oral dermabrasion. I often perform this procedure that includes dermabrasion around the entire mouth as a solution for lines over the lips and general "rumples" (a very technical term) of the general area. I have found that this procedure is the best solution for the upper lip lines that cause "bleeding" of lipstick. I also use dermal fillers to redefine the upper border of the lip. The combination of these 2 procedures makes an amazing difference in changing the aging appearance of the mouth and the immediate area surrounding it.

Branded Surgery Is Not Always the "Quick Fix" It Is Advertised To Be
|
New York, NY (July 16, 2009) - Have you seen the commercial for in-office procedures that will make your face wrinkle-free or sculpt your stomach, with no downtime and no scars? Lifestyle Lift, Lunchtime Lift, Thread-Tox, and Smart-Lipo, are just a few of the brand-name surgical procedures being marketed to the public as a cosmetic quick fix with a clever name. Brand name surgical procedures generally fit under two categories; first, those that are legitimately assigned the name of the person that has popularized the technique, for example Saldanha's lipoabdominoplasty or Furna's conchal setback technique, (either through the efforts of that same person or others assigning his or her name to it) and second, those that are simply created by someone to market a the procedure to the public. The first situation is usually a very legitimate situation and is often in the scientific literature, but not in the public domain. However, the second situation has become very popular for cosmetic surgery procedures. But are highly marketed procedures really the right choice when deciding on aesthetic surgery?
"In the wrong clinical setting, the results may not come close to the promises made in the advertising," says J. Peter Rubin, MD, a plastic surgeon in Pittsburgh, PA. "What is really indispensible for the best results, however, is not a specific procedure but the judgment of a board certified plastic surgeon who can match the right patient with the best procedure for them."
"These named procedures are used by the marketing entity to popularize the technique so that patients ask for it whether it fits their situation or not. This is a problem that can be very dangerous," said Dr. Al Aly, a plastic surgeon from Iowa, and a member of the Aesthetic Society's Body Contouring Committee. "A procedure is only as good as the hands that perform it."
"Patient safety and efficacy need to be the top priorities, not commerce or marketing," says Robert Singer, MD a plastic surgeon from La Jolla, CA and a past-president of ASAPS. If you decide a procedure is right for you, make sure you have done your homework, that the procedure has been fully explained, you know exactly who will be performing your procedure and that they are qualified to perform the exact procedure you are undergoing, and that you have thoroughly read and signed informed consent documents.
Facial surgery and body sculpting procedures are often marketed to the public with brand names. "One does not have to be a plastic surgeon to know intuitively that one facelift technique will not be appropriate for all individuals undergoing a facelift," says Sherrell Aston, MD, past-president of the ASAPS and member of the Society's Facial Surgery Committee. "The human anatomy and the aging process vary significantly from person to person. The so-called minimally invasive surgical procedures have gained popularity in all surgical specialties. For many procedures the work that is performed through the small incisions is rather extensive, and requires significant expertise on the part of the surgeon. A short incision facelift can give an excellent result, when properly performed for the appropriate patient."
"The issue of untrained or inadequately trained practitioners, some of whom are not medical doctors, performing cosmetic plastic surgery is an extremely serious patient safety concern," says Salt Lake City, UT plastic surgeon and president of the American Society for Aesthetic Plastic Surgery (ASAPS), Renato Saltz, MD. "For ultimate patient safety it is essential that the media and the general public be better educated about what constitutes appropriate training to perform operations such as facelifts, rhinoplasty, liposuction, abdominoplasty, breast surgery and cosmetic eyelid surgery." The demanding residency program that all plastic surgeons must complete before they can be considered for certification by the American Board of Plastic Surgery (ABPS) ensures that they not only acquire general surgical knowledge and experience but, additionally, that they master the principles, ethics and practice of plastic surgery. |
Featured Procedure: Perioral dermabrasion
|
Dermabrasion is a widely used method of refining and smoothing the facial skin via a controlled surgical removal of the top layer (the epidermis). Dr. Hand uses a procedure called perioral dermabrasion to remove the thickened, plaquish skin of the upper lip and to refine the wrinkles around the mouth caused by repetitive motion. Many patients needing this procedure are reformed smokers or may just have a familial tendency for the mouth to develop deep wrinkles. The best candidates for dermabrasion are lighter skinned individuals with no current acne issues.
The procedure is performed by Dr. Hand in our accredited Marin County outpatient facility under local anesthesia. Most procedures are finished in about an hour. The area to be dermabraded will first be covered with a numbing cream and then infiltrated with topical anesthetic. The dermabrasion procedure should be virtually painless. During the procedure, Dr. Hand utilizes a burr with diamond particles attached to a motorized handle to scrape away the top layer of skin. He will lightly abrade the skin to a level that is safe, making sure that the wrinkle or thickened area is dramatically softened.
Afterwards, a sticky, occlusive dressing will be applied that is to remain undisturbed until the following day when the area is examined by Dr. Hand. The dressing should stay on the surgical area as long as possible. Polysporin ointment may be used to keep the dressing flattened and sticking to the wound. After 3 days, the area may be gently washed and patted dry. It will be healing and forming new skin underneath the abrasion area. A product called Aquafor may be used to keep it lubricated and to relieve tightness. The new skin will remain pink for a period of weeks, perhaps longer. Camouflage makeup may be applied and used after the scabbing of the perioral area is resolved. The lower facial area should be protected from sun exposure until it has faded to the normal color of the skin. A hat and sunscreen are always advisable if the patient will be outside.

Cosmetic Surgery Vacations: Beyond The Sales Pitch

Ever thought about taking a "cosmetic surgery vacation"? Just imagine, while you're getting your face lifted or your tummy tucked, you could be having a glorious time in some exotic, out-of-the-way location -- right? The American Society for Aesthetic Plastic Surgery (ASAPS), the nation's leading organization of board-certified plastic surgeons specializing in cosmetic surgery, recommends against it. Here are some reasons why you may want to think twice, especially about traveling abroad for your cosmetic surgery:
1) You won't be ready for a round of golf. Most cosmetic surgeries involve restrictions on normal activities, and particularly exercise, for several days to weeks. So, while family members might enjoy the benefits of a resort destination while you undergo surgery, just remember that most vacation-type activities will be off-limits for you. 2) Sun bathing is taboo. If your idea is to recover from surgery lounging by the pool, remember that many types of cosmetic surgery require you to stay out of the sun until you are fully healed. Even then, you will likely be advised to protect yourself with sun block, sunglasses and a broad-brimmed hat. 3) Your doctor's credentials may not be what you think they are. Although there are excellent surgeons in many countries, it is often difficult to evaluate the training and credentials of surgeons outside the United States. Training and credentials vary, and there is no single international standard for quality such as, in the U.S., certification by the American Board of Plastic Surgery. 4) The surgical facility may not offer necessary safeguards. In the U.S., patients may choose to have surgery in an accredited facility that meets accepted safety standards for equipment, personnel and emergency procedures. Many facilities outside the U.S., or those within this country that are not accredited, may fail to meet even minimum safety standards. 5) Necessary follow-up care may be unavailable. Cosmetic surgery patients need to be seen by their surgeon at various intervals following the operation. Initial visits are to determine that recovery is progressing as expected, and later visits are to evaluate long-term results. If your surgeon is hundreds or thousands of miles away, there is no such continuing relationship. If for some reason you decide to have cosmetic surgery away from home, make sure that your surgeon has a professional colleague nearer to where you live who has agreed to see you for any necessary follow-up care. 6) Post-surgical problems are not easily handled. Serious problems are rare when cosmetic surgery is performed by a qualified, board-certified plastic surgeon. However, the results of cosmetic surgery are never 100 percent predictable. Problems, most often minor, which require surgical revisions or other treatment are always a possibility. In such instances, it is important that your surgeon is accessible. Patients who believe they can have surgery abroad and then, if necessary, find a surgeon close to home to handle any problems may find it tougher than they think. Most doctors are reluctant to handle problems, even minor ones, of surgeries they did not perform, particularly if they are unsure of what specific techniques were used in the initial operation.
7) Many qualified and reputable plastic surgeons arrange pleasant after-care accommodations, complete with private duty nursing, for their cosmetic surgery patients. This can make the recovery process easier and more enjoyable, whether patients live locally or come from out of town. However, promises of turning cosmetic surgery into a fun-filled vacation should be regarded with skepticism and caution. |
