ASK THE DOCTOR
My family is now complete, what options are available to rejuvenate my body and breasts?
In the past few years, the term "Mommy Makeover" was invented. Very simply, a mommy makeover is a combination of procedures intended to refresh areas of a womans body that have been altered after the birth of her children. In my practice, my patients are often concerned about lax skin and muscles of the abdomen and loose skin capsule with decreased volume of the breasts. The most requested procedures to address these concerns are abdominoplasty, liposuction and breast augmentation or lift. Sometimes the patient will elect to undergo dermal fillers at the same time as the procedure to avoid discomfort from injections.
I often will perform 2 surgical procedures on the same day of surgery. If multiple corrective surgeries are planned, I prefer to stage the procedures to 2 separate surgical days for patient safety and comfort.

Latisse Timeline and Comments

Just because it's interesting, I thought I would include the general timing as to when most patients see results. So far most are seeing an increase in length and thickness now that it has been has been exactly 8 weeks since my first group of patients started treatment. Everyone seems to be right on schedule for the expected results addressed by the timeline. Allergan, the makers of Latisse are reporting that once the initial 16 week results are obtained, the results can be maintained with 1 to 2 applications per week.
Comments I am hearing:
"Definately longer and thicker" Jyll
"My eyelashes are hitting my glasses. Do my lower eyelashes look too long??" Ardeth
"Someone asked me if I had false eyelashes on" Ivonne
Didn't wear mascara for awhile and happily noticed the much longer and thicker eyelashes when I applied it" Martha
Someone asked me if I had changed my eye makeup, my eyes looked beautiful" Lisa
"After 3 weeks, I think they are already longer and thicker" Dianna
This is great stuff, I'm using it FOREVER" Jeanie |

I am often asked "What is the most popular cosmetic surgery procedure performed at our office?" This month the national statistics came out so it is always fun to compare my practice to my colleagues. I would say that the most requested facial surgical procedure is eyelid lift and the most requested body procedure is liposuction in my practice. Even in this difficult economy, I have seen an upsurge in hylauronic fillers (Restylane, Juvaderm, and Elevass) and Botox. For the first time in years nationally, breast augmentation has taken over the top spot with liposuction following as a close second. Therefore, this month's featured procedure is breast augmentation. Although I usually include a before and after photo on this page, I have decided to post only a link to my breast augmentation gallery page, in order to respect those patients that do not wish to view nudity.
This month's interactive tool is a Botox simulator. This tool is similiar to last months Juvaderm simulator. If you missed it, you can find it in the March newsletter, found in the archived newsletter link. You can upload a digital photo of yourself or simply manipulate a models face to see the corrective wrinkle applications of the hyaluronic filler products.
Our favorite new product is still Latisse. Both Lisa and Cheri have seen great results from it. Lisa's eyelashes are much thicker and longer after 8 weeks. She is excited that she should have increased results for the next 8 weeks because she already thrilled with the results so far. All of our patients that have been on the product for 8 weeks are reporting wonderful changes. The funniest and most enviable was our client Jeanie that called to say she had one eyelash that touched her eyebrow. Lisa offered to pluck it for her. She declined!
A number of our patients have successfully investigated and qualified for CareCredit in this past month. CareCredit has a current offer through April 30, 2009 for 18 month, 0% financing for amounts from $300 to $25,000! Details can be found in this edition of our newsletter.
All the best,
Terry Hand, M.D.

Featured Procedure: Breast augmentation |
Dr. Terry Hand is recognized as a highly skilled and premier specialist in breast augmentation and breast restoration. Dr. Hand understands that the decision to enhance the breast size is a very personal one that can result in an improved body image. During consultation, you will be presented a detailed, informed presentation of all aspects of the breast augmentation procedure that is tailored to you individually. Techniques, surgical approaches and recommendations of appropriate breast implants, as well as what you can expect before and after surgery will be covered in detail.
Candidates for breast augmentation may have small breasts due to lack of development or a congenital abnormality in which one breast is larger than the other. A woman may seek to restore the size and shape of her breasts which have been lost due to pregnancy, weight loss or aging changes or the choice to enlarge the breasts may simply be due to the desire to enhance the appearance in and out of clothing.
Procedure Description
Breast augmentation is performed by Dr. Hand in our accredited Marin County outpatient surgical facility under general anesthesia. The procedure is done through a small incision that is placed in either the armpit or around the areola. The implants are inserted either completely or partially behind the pectoral muscle and breast tissue or in front or the muscle but behind the breast tissue. Implant placement is determined by Dr. Hand based on breast and chest wall size and shape, and your desired result. Our office utilizes both saline and silicone implants. Silicone implants generally produce the most aesthetically pleasing result and the most natural feel and appearance of the breast.
Recuperation and Healing
Immediately after breast augmentation, dressings are applied to the incision sites and a compression bandage is wrapped around the chest. The dressing is to be left undisturbed until the 1st postoperative visit with Dr. Hand the following day. On the 1st and 4th postoperative days, Dr. Hand will do a postoperative exam of your breasts and answer any concerns or questions you may have. At the 1st visit, another compressive garment will be applied by Dr. Hand that is to be worn day and night for 3 days to 2 weeks after surgery, depending on the position of the implants. On the 4th postoperative day, an exercise regime designed to move the implant in the newly created pocket will be instituted. The purpose of these exercises is to keep the pocket surrounding the implant intact, allowing for natural movement of the implant. Swelling will begin to subside after 1 week and after 3 weeks the breasts are close to the size they will remain.
Initial discomfort is easily controlled by oral medications. Activities requiring raising the arm above the level of the upper chest should be avoided for 10 days after surgery. Usually modified work and driving may be resumed in 3 to 4 days. A regular exercise regime may begin in 2 weeks.
Other Options
Breast augmentation is frequently accompanied by mastopexy (breast lift) when the natural breast has experienced a large amount of sag due to pregnancy, breast feeding, weight loss or aging changes. Liposuction of the torso or legs can often be combined with breast augmentation to redefine areas with exercise resistant fat accumulations. Abdominoplasty (tummy tuck) may be an option to remove extra abdominal skin and to redefine abdominal musculature.

10 Cosmetic Plastic Surgery Predictions For 2009 |
The American Society for Aesthetic Plastic Surgery (ASAPS), the leading national organization of board-certified plastic surgeons who specialize in cosmetic surgery, offers its predictions for cosmetic surgery in 2009. Predictions are based on interviews with leading plastic surgeons around the country. 1. Aesthetic Medicine has seen a dramatic increase in the diversity of the patients treated over the past decade and this trend is expected to continue to grow stronger than ever, with applications that cater to all people. The appeal of both aesthetic surgery and cosmetic medicine will continue to spread across the spectrum of our population, as plastic surgeons further tailor treatments to meet the sometimes unique needs of that expanding population. 2. The growth and popularity of cosmetic fillers (Elevess, Juvederm, Restylane, etc.) will continue to increase as products continue to evolve and new players enter the market. 3. As our population increasingly realizes the dangers and health consequences of obesity, the number of patients seeking plastic surgery procedures for body contouring after dramatic weight loss (abdominoplasty, lower body lift, upper arm lift, etc.) will rise in 2009.
4. Reloxin (an injectable form of Botulinum Toxin Type A) will gain FDA approval and compete with Botox (the most popular cosmetic procedure for the past 5 years) and other similar products may begin to enter pre-market clinical trials.
5. Consumers looking for a bargain on cosmetic procedures will unfortunately lead to an increase in horror stories about "discount injectables" bought offshore and cosmetic medicine and cosmetic surgical procedures performed by untrained or poorly trained practitioners.
6. Experimental techniques for non-invasive fat removal (SonoScultpt, UltraShape) as a future alternative or adjunct to liposuction (lipoplasty) surgery, will continue be tested in clinical trials.
7. Men will represent a growing segment of the aesthetic surgery market. (According to a February 2008 consumer survey commissioned by ASAPS, 57 percent of men approve of cosmetic surgery, and 20 percent would consider having cosmetic surgery. Seventy-nine percent (79%) of American men surveyed said they would not be embarrassed if people in addition to their family and close friends knew they had undergone cosmetic surgery.)
8. As the popularity of non surgical and minimally invasive procedures continues to grow; surgeons and manufacturers will develop new techniques and products that advance the science, produce even better results and lessen recovery time.
9. Cosmetic surgery "vacations" will become less popular as educated consumers understand the safety and risk issues associated with surgical procedures and travel. Surgery performed by board-certified aesthetic plastic surgeons, in accredited facilities where follow-up care is available will continue to be the safest option and the best value.
10. Following the trend in increased consumer sophistication regarding healthcare choices, board certification of practitioners, and accreditation of surgical facilities will play an even more important role in choosing a cosmetic surgeon.
