If I am trying to avoid surgery on my face and eyes for a little longer, what are my options?
Plastic surgery of the face has 2 components: the aging issues that cannot be corrected with non- invasive solutions such as dermal fillers (Juvaderm, Restylane, Radiesse & Elevess) and those that can be temporarily assisted by filling in stubborn lines or managing facial pleating with Botox.
Surgical solutions are probably the best answer in most cases for problems related to large skin folds because of laxity of the skin. Conditions such as fat pads around the eyes should never be treated with dermal fillers. Remnants of the filler almost always causes uneven bulging as the inital swelling subsides. An eyelid lift with removal and/or repositioning of the under eye fat pads is always a better solution with longer lasting results.
Fine lines, nasolabial creases (nose to corners of the mouth) and thin lips respond well to injectable dermal fillers. Pronounced jowls and/or a neck that has lost definition and angle can only be appropriately treated with a facelift procedure. |

Happy Valentines' Month! I was so pleased to reconnect with many of you after our last newsletter. Your support, comments and questions were greatly appreciated. Thank you! This month I added a blog to our website. I have lots of opinions and information, so what better way to share than a blog!
As I mentioned last month, Latisse is the big news on the plastic surgery forefront. Lisa and Cheri grabbed it the day it hit our office and have already started using it. They were even photographed without makeup for a pre-Latisse photo! We'll photograph them again every 4 weeks and look forward to photos similar to the ones posted later on this site, demonstrating the great results Latisse provides. This month our featured procedure is liposuction since the cooler months are always a great time to consider body recontouring. Liposuction continues to be one of our more popular procedures because it really does redefine areas of the body that seem to be unchangeable with diet and exercise. For most, it is a boost from the winter doldrums, making revisiting spring and summer wardrobes an event to look forward to. Liposuction results are an adventure for the patient and I to watch evolve. Very soon after the procedure, the silhouette is obviously redefined and within a few weeks the contour of the treated area seems to evolve almost daily. One of the really interesting parts of the today's internet is the availability of interesting tools to educate and entertain patients. This month a "click of a link" will bring you a tutorial on the Vaser liposuction technique. This ultrasound liposelection method has totally changed liposuction. Areas that previously were difficult to treat, such as the dreaded "back fat", are easily removed with spectacular results.
This month's article discusses "What is the primary factor in aging, genetics or environment?" The article reports some interesting aging predictors related to the effect of antidepressants, divorce and weight loss on the face. Although this article is interesting, I believe the top two factors in accelerated aging are sun abuse and smoking. My best recommendation, wear your sunscreen and trade cigarettes for dark chocolate!

Winter: A Great Time To Rejuvenate And Relax

Well it's official, winter is here. Although the cold, dismal weather never seems to last for long in Marin County, it is a great time for a tune-up (sorry, I couldn't resist). Many procedures are well suited for cold weather times because clothing tends to lend more coverage, sunshine on newly refreshed skin should be limited and most importantly, our social agendas tend to be a little less demanding. When the spring comes and our wonderful weather returns, it is always quite pleasant to feel refreshed and revitalized in appearance and spirit. What are my top 4 suggestions for this time of year? Out with the old skin and in with the new! I am an absolute believer in the Obagi skin care system, having seen dramatic results for a wide variety of my patients. I encourage most patients to start their skin care regime during these colder months in order to minimize sun exposure for the first 2 months. Our office offers the entire Nu-Derm line as well as the Obagi Blue Peel. The Nu-Derm line is a structured skin care system that renews the skin at a cellular level, decreases fine lines and dramatically evens out pigment changes. The blue peel does the same with one rapid peel that should be followed with Obagi products to maintain the refreshed skin. Lose an inch, gain a smile! Liposuction is one of the most popular procedures in my practice for several reasons: Most of us gain weight unevenly as we age. Fat deposits stubbornly occur in places we never thought could happen to us. Using the Vasar liposelection technique, areas such as the bra line rolls and thickening waists, hips, thighs or knees can be retailored to achieve symmetry with the rest of the body. A compressive garment does need to be worn for about 3 weeks following surgery, making this time of year a great time to wear loose fitting, casual clothing. Less is more! One of the pieces of advice that I give to all of my patients is to curtail most exercise and physically demanding activities for about 2 weeks after a surgical procedure. The reason for this is to avoid increasing your blood pressure and heart rate which often leads to more bruising and swelling. I am always amazed at how quickly recuperation occurs for those that allow their bodies to rest and recharge as advised. This cool, wet weather offers a great opportunity to rest without the temptation of outdoor activities for a short time. Turtlenecks are your friend! Facelift is another procedure that is often done this time of year. Although bruising is often minimal, a small amount will occasionally be noticed on the neck. This occurs because during a facelift the neck is redefined and often corrrection of neck banding is done . For that reason, the camouflage clothing route (the turtleneck), is often welcomed by those looking to disguise any postoperative bruising in the first few weeks after a facelift.

Featured Procedure: Liposuction |
Liposuction is a surgical procedure designed to sculpt and remove localized fat in areas that appear to be resistant to diet and exercise. Very often, because of the aging process, fat redistributes to locations that were previously never a problem. Liposuction targets these areas permanently. The possibility to gain weight still exists, so fat redistribution may occur with a large weight gain. Liposuction may be performed on the neck, the arms, abdomen, hips, back bulges, buttocks, thighs and knees. An appropriate candidate for liposuction is in good health and usually is of average weight with healthy, elastic skin. Some individuals have bulges in the neck or thighs that have been apparent since puberty, changing little with weight loss. Others, particularly when entering the 40's, will see increased abdominal fat and back fat that lingers. Often, genetically inherited fat accumulations can be altered positively. Small amounts of fat, removed from isolated areas, often create amazing results in the body's contour.

Dr. Hand utilizes Vaser liposelection, an internal ultrasonic instrument that is tissue-selective in that it targets unwanted fat while preserving other surrounding tissues. This technique also promotes smoother contours, decreased healing time and bruising. It is much more precise than older methods that used large cannulas and were not selective of the tissues that were disrupted. The ultrasonic waves produced by a special cannula tip, essentially rupture and emulsify the targeted fat cells, making them easier to aspirate with smaller cannulas.
Most patients can return to non-strenuous work in 3 to 5 days after surgery. Most exercise maybe resumed in 2 to 3 weeks. Some swelling and discoloration may persist for 3 weeks but a dramatic difference in contour is often seen immediately. The final result of liposuction can take 4 to 6 months during which time the skin tightens over the treated areas.

Latisse For Lush, Long Lashes! |
Latisse™ is the first and only FDA-approved prescription treatment for inadequate or insufficient lashes, proven to grow them thicker, longer and darker! The active ingredient in Latisse™ is Bimatoprost. Originally, Bimatoprost was used in the treatment of glaucoma. After noticing an increase in eyelash growth and thickness in patients receiving treatment, researchers noticed amplified growth and volume of eyelashes occurs because of an increased percentage of the affected hairs during the growth phase and that the duration of the growth phase seems to be extended. Latisse™ contains a small percentage of Bimatoprost and is intended for application only to the top of the upper lash line. Once treatment with Latisse™ is initiated, most patients begin to see increased length in 4 weeks with full results after 16 weeks: 25% longer, 106% fuller and 18% darker! The product is to be used only once per day in the dosage prescribed. No increased affect occurs because of more frequent or greater usage of the product. There have been few side effects. 4% of tested individuals had redness of the eye or an itching sensation. Most users found that these irritations went away once they became accustomed to the product or began applying it properly. There has also been reported an occasional darkening of the upper eyelid skin at the point of application because of increased melanin production in some users. The only patients that should avoid Latisse™ are those with allergies to the product and those with increased intraocular pressure may want to discuss Latisse™ with their eye doctor before initiating treatment. Latisse™ should not be used if you have an active eye infection or irritated skin on your eyelid. The FDA reviewed clinical study results to verify the identity, potency, purity, and stability of the ingredients, and demonstrated that the product is safe and effective for its intended use if used as prescribed. Since Latisse™ is a prescription product, our office will need to see you for a short, no-charge initial consultation to discuss if Latisse is right for you and to photograph your eyelashes to track your progress. After you have begun treatment, we would like to see you bi-monthly for several cycles to document your eyelash changes and to answer any further questions. ASPS Twins Study Finds Environmental Factors Trump Genetics In Facial Aging
In this study, Dr. Guyuron and his colleges studied 186 pairs of identical twins because they are genetically programmed to age exactly the same, and in doing so they discovered that, when it comes to your face, it is possible to cheat your biological clock.
Results showed that twins who had been divorced appeared nearly 2 years older than their siblings who were married, single or even widowed. Antidepressant use was associated with a significantly older appearance and researchers also found that weight played a factor too. In those sets of twins who were less than 40 years old, the heavier twin was perceived as older, while in those groups over 40 years old, the heavier twin appeared younger.
