We are happy to announce that we will sell pizza on Fridays.
Please see the order form below with meal options and instructions. Our first PIZZA FRIDAY will be on January 7, 2011.

Please send cash (exact change ONLY) or Check (made payable to CIS PTA) by Thursday mornings.
You MUST pre-order. We will not order extra pizza. Place your order form and payment in a SEALED envelope and return to classroom teacher. Please fill out one (1) form for each student.
If you are writing a check, you may write the check for multiple students. Please list the names and grade level for each student.
