Please check your child's backpack today. Our school nurse, Bonnie Kaufman, is sending home a permission form for your child to receive the influenze/H1N1 vaccination.
If you would like your child to receive this vaccination at an upcoming CIS shot clinic, the permission form must be signed and returned by Thursday, October 7th.
Once the forms are returned, the order can be placed and the shot clinic will be scheduled within a 2 week period after all forms are received.
