In the next couple of issues of RDNews, we will dedicate some paper to reviewing the importance of properly documenting your paperwork.

How complete is your paperwork?
 
In this world of "documentation is everything" and "if it isn't written, it didn't happen", how does your paper trail look?
 
 Recently, one of our RDs, Elizabeth Brown, was put to the test!  A client requested reassurance and verification of RD hours worked, so the paper trail was followed. An online review of her visit reports for the months of March, April, May and June was conducted. Her On-Tap timecards for the same months were checked, and upon evaluation, Elizabeth had matched almost 100% with her timecards and visit reports. Her visit report times matched her time-in and time-out on her timecards. She also included important details in the comments section of her visit reports to discuss what she had done that day. She included a count of total residents seen for the day as well as what kind of assessments were done.  Elizabeth's online calendar matched her timecards and visit reports for that extra back-up. 
 

Due to Elizabeth's conscientious attention to detail, and appropriately following company policy and procedure for reports and timecards, we were able to provide the reassurance to the client on the RD hours worked. Thank you so much to Elizabeth Brown for all of her hard work and dedication to HMC!
RD Tip
  • Timecards and calendars should match 100% with visit reports
  • Visit report should be done every time you walk into a facility even if you are just there a couple hours, for a short meeting, for vacation coverage, etc.
  • Email your visit report to the Administrator and DSS. If email is not the best way to communicate with them, provide them a hard copy of the reports. Best practice is to provide a hard copy and email.
  • Provide detailed information on the visit report with number and type of residents seen/assessed, and what other dietary oversight was done. Your documentation shows what you accomplished for the day.
  • Refer to the staff website to download/print the policy and procedures for completing RD reports and On-Tap timecards.

Your visit reports can help us in surveys, help clear up invoice questions, and help trigger your memory for what you did on a specific day and it can also help our Senior Staff support you in the field.
