Open Streets Brings Thousands out to Play
 
On August 1, tens of thousands of people replaced car traffic with physical activity on 8 miles of Chicago streets for Open Streets, an event that encourages fun physical activity in Chicago neighborhoods.

Residents rode bikes, walked and ran along the route, which passed through the Logan Square, Humboldt Park, Garfield Park, North Lawndale and Little Village neighborhoods.

This month Erie is spotlighting a multitude of exciting news and events. Our Lending Hands for Life Program (LHL) has been selected as the winner of the National Quality Center's 2009 Award for Performance Measurement. Erie's Tu Diabetes Program is underway at another location, Erie West Town Health Center. Erie Program Receives National  Honors 
The Lending Hands for Life program LHL teamat Erie Family Health Center has been selected as the winner of the National Quality Center 2009 Award for Performance Measurement. 

The award honors Erie for significantly strengthening its ability to measure the quality of HIV care and demonstrating a strong ability to collect and share data. "This award shows that we have the clinical and Quality Improvement (QI) leaders within Erie who can put theory into practice. They measure, they make a plan to improve a challenging issue, and then they measure again, "says Dr. Lee Francis, President and CEO of Erie Family Health Center. "Building on 20 years of our program, we have shown even more fantastic results under new leadership in just the past three years."

Gustavo Hernandez @ Erie Helping Hands
Making an impact. When Gustavo Hernandez received his high school diploma, he knew his primary ambition was to find a career to contribute to the well being of others.
 
His first step toward this goal was applying to the Illinois School of Health Careers (ISHC) to receive his certification to become a medical assistant. After a year of tough training and coursework at ISHC, Gustavo was required to complete a month long externship. He immediately recognized Erie Family Health Center as a leader in community health and after extensive interviewing was selected to fulfill his certification requirement at Erie. Weeks later, Nori Sanders, Manager of Clinical Operations, offered him a full time position at Erie Helping Hands Health Center.

"I knew from day one Gustavo is a caring person," says Nori. "He truly cares about the patients, their medical concerns and overall experience while at Erie, as well as the needs of our providers and team members."  Gustavo's dedication to his position, patients and fellow staff are rooted strongly in his dedication to serve. "I have always enjoyed helping people and this career has encouraged me to stay optimistic even during the hard times," he says.

Erie West Town Health Center
The spotlight is on the Tu Diabetes Program at Erie Family Health Center's West Town location.
 
An innovative and effective way to treat diabetes is now underway at Erie West Town. The program, TuDiabetes, utilizes a unique group-based medical approach.  Patients meet together monthly and participate in group education sessions lead by a multidisciplinary team, as well as individual medical visits with a medical provider.  This program takes a different approach in addressing the ever-growing diabetes epidemic in the United States, specifically on Chicago's west side.  Ana Cesan, the Diabetes Care Coordinator at Erie Family Health, says, "it incorporates social support and problem solving skills in an environment more conducive to dealing with the complex psychosocial and medical issues involving diabetes." 

TuDiabetes was originally piloted at Erie Humboldt Park Health Center, a neighborhood with a 30% higher rate of diabetes-related deaths than in Chicago. Also, 14% of the residents in Humboldt Park have diabetes compared to 5% of all Chicago residents. In response to these alarming statistics, Erie decided to use a group-based program to help combat and treat diabetes on the west side of Chicago.
