Question: At least once a month at our pedorthic retail store, we see a customer that needs a shoe that will fit a charcot shape foot on while having a normal contralateral foot. While refurbishing a relasted PWM Leisure Time DX2 shoe and its mate for a customer for a wedding he was attending, we were discussing options to have additional shoes. He has a limited budget so he needs shoes that are not only functional but durable. When I discussed this need with the orthotist that sees her TSD patients at our store, she mentioned that APIS/Boxerdog brand advertises a charcot shoe in its catalog.  Have any Pedorthic Newswire subscribers had experience with this shoe and if so, what has been the process of ordering and the ultimate satisfaction with the fit, function, and durability?

Question: We have a pharmacy and would like to learn if we are able to fit Therapeutic Footwear for people with Diabetes and inserts? Are we able to hire a C.Ped or equivalent and fit these patients under a PHARMACY BENEFIT (not DME.)

Response: The Therapeutic Shoe Benefit is a Medicare Part "B" benefit, where as pharmaceuticals are a Medicare Part "D" benefit. You must have a Part "B" (DME) contract to bill. As to professional requirements, Medicare DMEPOS Quality Standards (Page 4) state the following: "Professional personnel shall be licensed, certified, or registered AND function within their scope of practice as required by the State standards under which the professional is licensed, certified or registered." If you touch a patient (professional personnel), you must follow these standards. As to Certified Pedorthist vs Certified Shoe Fitter, you need to check with your State statutes. Some Licensure States have exemptions for Licensed Pharmacists, but Medicare still requires everyone else to be Certified, Registered, or Licensed as stated plainly above. Question: I posted this question about neutral running shoes months ago. The only response that I got was from Jeff Rich. While he offered excellent advice on sandwiching a firm lateral wedge into the sole of the shoe, I was hoping someone from one of the shoe companies would offer a response.  I have been more than perplexed as to why a shoe that is categorized as neutral, would have a thicker and firmer medial outsole and a thinner softer lateral one?

Many of these shoes are basically a medial wedge/pronation control shoe right out of the box. As such, many of the cavus or under-pronators (for which the shoe was designed for) actually have their functional deformity increased with these shoes. The theory may be that making a super soft eva or gel crash pad, it may absorb more shock? It makes no sense to have it collapse like a flat tire. I have seen this in certain shoes from Brooks, Nike, Asics and New Balance, so it isn't an anomaly to be blamed on one design or company.

As Jeff Rich indicated, I have also fixed more than a few of these shoes for desperate patients. I would also like to ad that most of these have been in cavus orthotics with lateral posting, reduced medial arch and a drop for the 1st ray. In spite of that, these shoes were still over-powered due to the lack of firmness and stability of the lateral heel. If you need a simple visual aide, think of this. Let's say you were fitting a CMT patient for an AFO. What would the effect be if you posted it medially with a 6mm thick wedge of 60 durometer eva, then posted it laterally with a piece of cloud crepe or firm Plastazote?

It would tip into varus, which is the exact opposite of what we are trying to control. I hate to sound like a broken record, but I can't possibly be the only one dealing with this. Certain models have progressively gotten worse over the last 2 years. I am getting tired of having to explain to a patient that has a $100 + neutral shoe that causes them to tip into varus and the shoe is less than a month old, sometimes newer. If any designers or product managers are listening, I have a question. Please let me know if there are any plans to go back to basic principles with a neutral shoe. Simply make the medial column almost a mirror image of the lateral column and see how that works. That is the true foundation of a neutral cushion shoe.

That simple design principal isn't sexy and it isn't cool looking.....BUT a large percentage of runners and non runners alike would benefit. I am not big on the minimal shoe craze but when I hear anecdotal stories of people that used to run in heavier structure shoes and their symptoms reduced when switching to a more neutral cushion shoe. I have to wonder, is this due to how great these new-fangled shoes are? Or, is it due to them being previously locked up in a shoe with too much pronation control?

Response: As much as I like recommending running shoes to my patients I have to remember that the majority of the cushioned (neutral) running shoes have a crash pad in them.  Not all of the shoes that have a crash pad have a posted medial midsole.  The Nike Air Pegasus has a standard EVA midsole.

In working with patients with a cavus foot type or lateral instability you may need to stick with a good walking shoe.  One shoe that I like is the New Balance 927/928.  This shoe has a medial/lateral roll bar in rearfoot to better help stabilize the foot, and is neutral on both sides. These type of shoes are more appropriate for patients with CMT I feel.

Question: We have an 80 yr. old Medicare patient with a LLD of 3 in. and a foot length discrepancy of 1 size. From a clinical standpoint there is going to be an issue with proper foot (specifically ball) alignment if a prefabricated pair of therapeutic shoes will be dispensed with the same size for both feet shoe. The Medicare LCD for therapeutic shoes states the following: A custom molded shoe (A5501) is covered when the patient has a foot deformity that cannot be accommodated by a depth shoe.

  

Is a foot length discrepancy considered a deformity that warrants a pair of custom shoes for a diabetic patient? Must we supply 2 pairs of prefab shoes instead & who is responsible for the cost of the 2nd pair? Will Medicare pay for 2 prefab shoes in the above situation? Could the LLD be taken in to the equation for custom shoes?

Response: I had to have a discussion with my billing department about this since I wasn't positive myself. I was guessing that this patient was diabetic based off of you're A-code. I was told that indeed, Medicare would not consider the discrepancies alone a medically necessary reason for a pair of custom shoes. Since the patient can be fit into a mis-mated pair and have the lift added to them I was told this would be the optimal choice. We would bill the 3 units for the shoe lift and two pair of shoes, mis-mated, one pair A5500 and one pair L3257, the latter of which will not be covered. We would have the patient sign an ABN and upon Medicare's denial for the second pair they would either be charged to the patient or written off by us. If you use a shoe company with a higher margin, the second pair could be made up for. (i.e. Propet) If the patient is not diabetic then custom shoes would not be covered by Medicare anyhow. I agreed with NJ C. Ped. other than according to my Brannock, a whole size is around ¼" discrepancy. A shoe like APEX may allow for the ½ size attempt to match but others may not have enough clearance. Obvious care must be taken to be sure of fit. If difficult to tell where the toes are in the shoe, a little baby powder on the end of the inserts before trying the shoes on can help see where things lie when they are taken off again. New York soda ban won't work, marketing experts say

Comedian Jon Stewart drank from an oversized soda cup on his show as he bemoaned the New York City mayor's plan to ban the sale of super-sized sodas from ballparks and other thirst-inducing places. And, Stewart complained, the proposal did what little else could - got him to agree with conservatives.


In fact, Mayor Michael Bloomberg's idea to ban the sale of sugary drinks (not juices or milkshakes) over 16 ounces from sports arenas, restaurants and other spots has many people complaining about the nanny state. Diabetes Drug May Increase Bladder Cancer Odds

People with type 2 diabetes taking Actos (pioglitazone) to control blood sugar may put themselves at risk for bladder cancer, according to a new Canadian study.

4 cups of tea a day can cut diabetes risk

The best way to slash the risk of developing type 2 diabetes is to drink at least four cups of tea a day, a study found.

A study of European populations found that people in countries that drank four cups a day - the British average - had a 20 per cent lower risk of developing the illness.

The study found that benefits seemed to be most obvious among heavy tea drinkers as drinking a mere one to three cups a day didn't lower the risk. Read the full article here.

Exercise video games offer fewer health benefits than many assume

Three-quarters of Americans believe video games that involve physical activity can be part of a fitness regime, according to a recent study by the UnitedHealth Group, a health insurance company in Minnetonka, Minn.
