NEW QUESTIONS AND COMMENTS
Question: I had a patient coming in for diabetic footwear. The patient diagnoses states the following: "Type 2 Diabetes; apparently cured with weight loss associated with bariatric surgery."
Medicare's Statement of Certifying Physician states;
"This patient has diabetes mellitus" not "has a history of diabetes".
These patients still have all the other qualifying foot conditions of diabetes. Do they qualify for the TSB? Thank you,

Question: I started reading Senate Bill S. 2125 and by page 2 my head was already spinning. Is there anybody who read this bill that understands this and can put it in simpler words? How would this impact our fellow Pedorthists? Thank you,

Comment: Attention all C.Ped's regarding Senate Bill S. 2125. My first point: the post on Senate Bill S. 2125 is an example of what we can expect with the government taking over health care. I think this bill only applies to products billed to Medicare. I am not a lawyer, however after trying to interpret the legal speak, I think this bill basically states all persons, no matter what credentials, will not be allowed to dispense any type of DME goods unless it is out of a Medicare accredited O&P facility. That means all C.Ped's, DPM's, PT's DC's, MD's will not be allowed to fabricate, dispense custom made or pre-fabricated orthotics of any kind, or soft goods of any kind unless they are employed by a Medicare Accredited O&P facility. People who run wholesale labs will not be able to sell their product to anyone expect a Medicare approved O&P shop. This will leave a lot of people out of work. I think this bill only applies to Medicare now but it is just a matter of time.
My second point: Those of you who think you and your patients will be able to purchase private insurance under nationalized health care without your practice feeling the effect are in for a rude surprise. Insurance companies are like any other company. They have to make a profit to stay in business. Of course they do. If they do not have adequate cash flow and adequate profits they will go out of business. So let me ask you this; how does a private company that has to make a profit to stay in business, compete against a government entity that does not have to make a profit? The answer is they cannot! The government can and does operate with a huge deficit and if they need more money to operate government run health care they simply take it from us (via higher taxes) or print more money. My prediction is if nationalized health care takes full effect in 2014, private insurance companies will be out of business in six to 18 months. Could you compete with another practice in your town that offered their products for FREE, had unlimited funding and did not need to make any profit to stay in business? If your answer is yes, you have your political party blinders on. This is not about politics!
I am now hearing rumors of Evidence Based Pay for physicians under nationalized health care. The doctors will not be paid by the government until the patient signs a form stating that the doctor was able to fix the problem that patient needed medical help for originally. This is a nightmare waiting to happen. How long it is going to take the doctors to get paid? Please follow up with more posts. My fear is many health care professionals are be legislated out of a job.

Question: With online shopping growing each quarter, I am curious to see what effect online competition has had on other store's pricing of shoes carried by the store and online vendors, including the brand's website. We are considering moving our pricing closure to MSRP but charging for anything we do that cannot be done by online vendors, i.e. adding heel wedges, volume fillers that we used to include in our "pedorthic retail pricing". -John Mishasek, BOCPD, CPed Response: That is exactly what we started doing about 1.5 years ago. We even charge for tongue pads. Do you get them for free? We haven't found anyone to supply fitting modalities for free but if you know of some company, please share. Then there is our time and expertise for which we deserve compensation. Pedorthists are hard-working professionals who are used to accepting low wages because we've given too much away. Some of us are waking UP!

Response: We have been affected by MSRP and MAP pricing online. We have updated our prices to be more competitive in spite of our higher cost of doing business. This has adversely affected our maintained margin. To compensate we are always on the lookout for exclusive and unique higher margin products not sold online. We have always charged for pedorthic modifications that affect balance, alignment, gait, and posture. We do not charge extra to make a shoe fit better (tongue pads, heel grips, etc). In order to compete going forward we must work together to buy and sell products not subject to MSRP and MAP pricing. The key to our future is to continue to provide each and every customer with unique experience and significant benefits versus internet and regular retail competition.

Response: We gave up Medicare also, and the company has been better from that change. I was interning a new LC.Ped when we made the decision to stop providing diabetic shoes. She passed her test, became certified, and became licensed in Ohio and works out of a pharmacy 30 miles away. She comes in 2 days a week and sees diabetic patients at our location. Medicare said if they do not have a sign on our door, she can rent space from me and it is seen as a convenience for the patient so Medicare was OK with it. She will also go to the patient's home. Problem solved! I still make all my old patients custom diabetic foot orthoses, and I get to chat with them and keep in touch.
While all this was going on a good friend of mine bought out closed Foot Solutions stock and carried out their lease in a high rent area. I talked to her about opening her shoe store next door to me when her lease was up. She is a LC.Ped also and sells comfort shoes like Z-Coil, Brooks running and assorted Drew and Aetrex. So we have created this Pedorthic Center around us without really trying! We are marketing to our doc's as a united group that can handle any Pedorthic problem they send our way. And have come up with a great name for our center, (I haven't got it locked in with the state yet so I won't tell you!)
So, I make the custom molded orthoses, sell orthopedic shoes for the hard to fit foot, cast for custom shoes. Our LC.Ped sells her diabetic shoes and is a regular standup comic so when she is here it is a day of laughter. It is strange how things evolve, last year this time I was ready to give it up and get a real job and now a year later we are a team supporting each other and each other's business. I guess the moral is to never turn a new pedorthic student away, you never know how they may help you in the future! Also, others in our profession are friends, not the enemy! Keep an open mind and work together with the other LC.Ped's is your community. It has certainly worked for me!

New York City was named the top city in the world for global competitiveness Monday, edging out London and Singapore, according to a report by the Economist Intelligence Unit. Paris and Hong Kong tied for fourth and fifth, while Washington, Chicago and Boston were the other U.S. cities to make the top 10. The report measured competitive across a variety of categories including economic strength, financial maturity, social and cultural character and global appeal. New York emerged as the leader because its scores were high virtually across the board. Full Text

High Heels Can Bring on Ingrown Toenails

High heels and other snug-fitting or pointed shoes are a leading cause of ingrown toenails, according to podiatrists. By putting constant pressure on the toes, the big toenails can grow into the skin and cause an infection, the experts at Loyola University Health System said, yet many women are unwilling to trade in their heels for more practical shoes. "Ingrown toenails can be painful, but many women are willing to cope with the discomfort in order to continue wearing their high heels," Rodney Stuck, a professor of podiatry medicine, said in a university news release. "However, more serious complications can arise and cause permanent damage to the toenails, if they are left untreated." Full Text

From landing you one step closer to 'the one' to living inside an actual shoe, fairytales are written to make you believe in the power of the shoe. They'll have you think that the right slipper can make or break your day - and sometimes even your life! But in real life, most shoes only end up breaking our backs. Sometimes finding shoes that aren't uncomfortable, ugly, and don't cause blisters seems impossible. | APhA, ADA announce new collaboration

A new partnership aims to educate pharmacists, other healthcare professionals, caregivers and patients about diabetes and about how pharmacists can work with physicians to help patients manage the condition. The American Pharmacists Association, the APhA Foundation and the American Diabetes Association announced the collaboration Friday, saying it would develop new resources and also promote existing programs, such as the ADA's Stop Diabetes Movement and the APhA's Pharmaceutical Care for Patients with Diabetes certification program. Diabetics should lift weights before cardio: study

People with diabetes may have better blood sugar control during workouts if they lift weights before doing cardio exercise, according to a new study by Canadian researchers. It's important to define the best way for people with type 1 diabetes to exercise so that blood sugar doesn't drop too low, yet they can still reap all the benefits of aerobic exercise, Dr. Ronald Sigal, an endocrinologist at the University of Calgary in Canada and lead author of the study told Reuters Health. Those with type 1 diabetes, a condition in which the body does not produce its own insulin, a hormone needed to convert food into fuel, risk low blood sugar during exercise. Blood sugar that drops too low can lead to poor coordination, unconsciousness or even coma. Full Text

Regulators consider waiving prescription for diabetes, cholesterol and other widely used drugs
Some of the most widely used prescription drugs, including those to treat cholesterol and high blood pressure, could be available over the counter under a new proposal being weighed by government regulators. Food and Drug Administration officials said Wednesday they are considering waiving prescription requirements for certain drugs used to treat ailments like diabetes, asthma and migraine. Driving the move is a wave of computer technology, including touch-screen kiosks found in pharmacies, designed to help patients self-diagnose common diseases. Full Text

Survey Shows People Have Little Time for Healthy HabitsLack of time seems to be the key reason why only 12 percent of American adults regularly practice such healthy habits as eating right, exercising, and brushing and flossing their teeth, according to an American Heart Association (AHA) survey. The survey found that 80 percent of respondents said they struggled to eat at least nine servings of fruit and vegetables a day, and about 60 percent said it was difficult to get the recommended levels of exercise -- at least 150 minutes a week of moderate activity such as brisk walking. The survey also found that 25 percent of respondents don't brush and rinse twice a day or floss at least once a day, as recommended. On the positive side, however, the survey showed that 90 percent of respondents said they did want to improve their health. Full TextSource. | Pedorthics is a professional field covering the design, manufacture, modification and/or fit of footwear. Pedorthics encompasses shoes, orthoses and foot devices that prevent or alleviate foot problems caused by disease, overuse, injury or congenital defect.
