Question: With online shopping growing each quarter, I am curious to see what effect online competition has had on other store's pricing of shoes carried by the store and online vendors, including the brand's website. We are considering moving our pricing closure to MSRP but charging for anything we do that cannot be done by online vendors, i.e. adding heel wedges, volume fillers that we used to include in our "pedorthic retail pricing". -John Mishasek, BOCPD, CPed
Question: I recently read that, for diabetic footwear to be most effective, it must be worn at least 60% of the time. All reported rates of compliance fall well short of this level. The style and appearance of the shoe have been commonly blamed for this poor compliance. As a CFTS or C.Ped, do you feel that the real reason for this low compliance is because of the appearance of the actual shoe?

Comment: We dropped out of Medicare and all other insurances summer before last. About 10% of our Medicare TSD patients are getting their shoes by self pay now. We do provide them a Medicare form that is used by beneficiaries to submit claims for direct reimbursement (CMS-1492S) but the DMEPOS for our region C must have an internal policy to find the lowest reimbursable item on receipt and send out a check for 80% of the Medicare fee schedule for that one item, hoping that the beneficiary will accept the underpayment and go away. Fortunately we have a large retired military population here that fought the good war, and we have prepped them on how to appeal and make CIGNA DMEPOS miserable with appeals all the up to the ALJ level.
We tried to be a contracted supplier to the largest Medicare HMO in town but they would preauthorize TSD services then not pay us for what they authorized. We spend so much time chasing payments that we dropped the contract after six months. I hear that Kaiser Permanente can be a good payor but then I have heard from other KP TSD suppliers that you have to constantly remind them to get paid too.
One of the largest Medicare TSD suppliers in town (a large O&P practice) just stopped offering TSD services this month so we are seeing an influx of new patients looking for someone to bill Medicare. We offer them the CMS-1492S option but tell them how CIGNA will try to cheat them out of full reimbursement. And the shoes being put out by places that have no shoe inventory are often ill-fitting and limited selection and fit by non-qualified office staff, so the prevention of diabetic complications has bottomed out here.
Unless one has the staff to chase orders and chart notes and payments and answer audit requests all of which reduce thin margins to negative profitability per transaction, I do not see how anyone (pedorthist, podiatrist, orthotist, prosthetist, or pharmacist) in their right mind would bill insurances for TSD services. Non-participating pricing above the fee schedule is not the solution as then you have to collect full payment up front, which many beneficiaries do not want to do. That is why we are helping only 10% of our previous beneficiaries now.

Comment: I would like to share this new piece of information: Senate bill S. 2125 - A bill to amend title XVIII of the Social Security Act to modify the designation of accreditation organizations for orthotics and prosthetics to apply accreditation and licensure requirements to suppliers of such devices and items for purposes of payment under the Medicare program, and to modify the payment rules for such devices and items under such program to account for practitioner qualifications and complexity of care. Senate bill S. 2125, which is in the first step in the legislative process, is co-sponsored by Senators Olympia Snowe (R-ME) and Chuck Grassley (R-IA).

Amazon Reigns at Top of Retail Customer Satisfaction Index Customer satisfaction with retailers at the national level inched up 0.1% to 75.8 on a scale of 0 to 100 for the fourth quarter of 2011, according to a report released by the American Customer Satisfaction Index (ACSI). The Q4 gain brings the yearly change for 2011 up to 0.7%, a mild improvement that is in sync with a very slowly recovering U.S. economy. "The good news is that customer satisfaction continues to climb, which has a positive effect on consumer demand and growth," says Claes Fornell, ACSI founder and author of The Satisfied Customer: Winners and Losers in the Battle for Buyer Preference. "Still, the ACSI improvement is tepid, and much of the fourth quarter gain comes from positive consumer reaction to lower gas prices. This is a story that hits - shall we say - close to home. Most women like their high heels, always have, always will. But some women's shoes aren't just high, they're downright punishing. Doctors say high-heel lovers may end up paying for it with foot problems. I'm kind of boring in the shoe department. But a lot of women are sporting five, six, seven inch heels, or crazy-high stilettos. Several of them work right here at FOX 5. Australian researchers studied a small group of young women - around 25 - who wear high heels. They found the shoes changed their alignment, their muscles and tendons, even the way they walk. But, oh, how we still love them. They look so good, "I love heels and I love them high, they have to be high. If they're not high, I don't want them," said Joy. "High heels, they're a must. Increasing Shift From Islet Antibody Positivity to DiabetesThere is an increase in both the prevalence and levels of islet antigen-2 and zinc transporter 8, as well as in autoantibodies, in newly diagnosed type 1 diabetes patients during a period of rising disease incidence, according to a study published in the March issue of Diabetes. Anna E. Long, of the University of Bristol in the United Kingdom, and colleagues analyzed autoantibodies to insulin (IAA), glutamic acid decarboxylase (GADA), islet antigen-2 (IA-2A), and zinc transporter 8 (ZnT8A) by measurement with radioimmunoassay of sera collected from children and young adults with newly diagnosed type 1 diabetes between 1985 and 2002.
