Question: My question is about insurance billing.  The store at which I work has billed for diabetic shoes and custom orthotics for a number of years. Like many of you, we have come to a point where the difficulties of obtaining reimbursement are starting to outweigh the benefits. The time and effort spent on chasing down prescriptions and certifying statements, checking coverage's for patients' insurance with whom we are not contracted (which of course means they will not "guarantee coverage" even though they give you a quote over the phone), re-submitting kicked back claims, etc. etc. All of these have reduced our margins to the point where we are asking is it viable to continue?  To those who have dropped billing, what percent of patients have you lost and do you see them return if you adopt a "pay out of pocket and we will bill on your behalf" policy. If you have continued to bill insurance, has anyone found having numerous insurance contracts help buffer the negative aspects of billing that we all deal with? We currently are contracted with one insurance company and that has proven to be quite smooth, hence the question. Thank you for your time.

Question: When billing Medicare for diabetic shoes is the initial visit for evaluation and sizing considered the date of service or (if ordered) the day the shoes are delivered to the patient?

Response: The date an item is fit and dispensed to the patient is the date of service, not the date on the prescription or the date you started work.  The delivery date is the date of service.  This applies to Medicare as well as most, if not all, other third party payers.

Response: Delivery date is the date of service. Only delivery of the covered item can be considered a delivery to file a claim.  Evaluations can result in no orders for many reasons (orders not received, orders received but inaccurate for billing purposes, patient failing to return for delivery, etc.) in which billing of non-custom shoes or direct mold inserts would not be appropriate.

Question: I am part of the Medicare Therapeutic Footwear Program for diabetics. The processing company I'm currently using is changing how they do business making it very difficult to stay with them. Therefore, I'm looking for a software program I can use in my store that: 

- Can access Medicare to verify eligibility of my diabetic customer.

- Can submit the sale to Medicare for payment.

- Can access secondary insurance companies (e.g. Blue Cross/Blue Shield) to verify eligibility.

- Can submit the sale to the secondary insurance company for payment.

 

Does anyone know of any software package that does all of the above or can name a company that will do all of the above and only collect their fee when I receive complete payment from Medicare and the secondary insurance companies. Thank you.

Response: We used a software billing program called DME Free. While it was not actually free, it was very reasonable priced.  Claims submitted through their software are aggregated and then all submitted to a clearinghouse (small fee charged per claim to do this) as this is the way smaller suppliers have their claims processed.  Software edits in DME Free will catch some mistakes but cannot catch other mistakes (wrong HCPCS entered, wrong quantity entered, mistake in patient's actual home address on file with Medicare, etc.)  So it takes a very detail oriented person to enter claim data.

 

Relative to specific issues mentioned in posting.  Medicare has a method to determine if a beneficiary is still covered by Medicare and not enrolled in a Medicare HMO and lets you print out a hard copy to put in the chart.  Do not expect the software to do this for you.  DME Free software submits claims (not sales) that all the correct billing information has been entered, and some claims will cross automatically over to major plans (BCBS, etc.) but your billing staff (yourself?) will need to verify crossover or submit a paper claim to the secondary payer.   Annual deductibles and secondary coverage limitations will need to be reconciled and patient balance billed.

 

We knew pretty well from years of billing which plans paid the coinsurance and which did not so that we could collect at time of delivery the 20% amount of claim.  In a thin margin area like TSD, collecting coinsurance at delivery while beneficiary is there means not having to generate a hard copy invoice multiple times to collect the amount that hopefully represents the gross profit on the delivery.  

 

Your language of referring to the deliveries as a "sale" indicates that you are very retail oriented, which is a little scary when one dealing with Medicare.  I recommend that you learn and use the lingo of being a Medicare supplier.  You file claims, not sales. You are reimbursed for delivering product to a beneficiary.   If one is going to be in a Medicare supplier, one should get familiar and use the terminology so when you interact with your DMEPOS staff, you can understand what they are saying to you.

 

It is a real challenge to wear both hats (retail and supplier) as the requirements of success in both areas are very, very different.

Kent State, Ohio Podiatric School Are Becoming One

Kent State University and the Ohio College of Podiatric Medicine in Independence plan to come together into a single, academic entity as early as this summer. Both institutions are collaborating to allow for a smooth transition. An oversight committee, comprising an extensive group of people from both institutions, is overseeing the transition. Six transition workgroups are also working to identify key transition issues and will provide analysis and recommendations in areas from academic affairs to marketing. "We are putting careful thought into this transition process to make sure there are no bottlenecks when the college formally becomes a part of Kent State," said Kent State Senior Associate Provost Tim Chandler, who is co-chairman of the oversight committee.

Following litigation against New Balance, Reebok and Skechers over their toning shoes, a plaintiff has filed a class action against Target alleging that the retailer's "TrimStep" footwear was deceptively advertised. The suit, filed on behalf of a national class in Minnesota state court, challenges Target's claims that its shoes "encourage muscle toning in the legs, improve posture, reduce stress on feet, knees and back and promote a healthy, active lifestyle," according to a report on the Association of Corporate Counsel.

When NHL star Mike Cammalleri slipped them on for the first time, the feeling was "primitive and powerful." He began darting around the artificial turf at the Montreal Canadiens' training facility in Brossard, Que., testing their worth. He was convinced they could make him stronger. Now the recently traded Calgary Flame has become the face of the adidas Adipure Trainer, billed as the first minimalist shoe designed for working out. It's the latest entry into the booming market for footwear with a less-is-more kick. Mr. Cammalleri was an early convert. About a year after that initial test run, "If I'm training, I'm wearing them," said the 29-year-old, known for his rigorous workout regimen. There are signs the shoes are verging on ubiquity. For runners, the trend away from tricked-out, uber-supportive shoes and toward minimal footwear has taken off in the past few years, turning a fringe product into a roughly $2-billion annual cash cow. The New Balance Minimus, Nike Free, Reebok RealFlex and Vibram FiveFingers are among the slew of stripped-down shoes - flexible enough to be rolled, scrunched and squeezed at will - praised for triggering small, neglected foot muscles.

More than 1.3 million diabetes patients not offered vital tests

An estimated 1.3 million diabetes patients are missing out on vital and potentially life-saving health checks, the BBC has learned. NHS figures reveal fewer than 10% of patients are offered the full series of tests in some areas of England. Health Minister Paul Burstow said the situation was "outrageous" and "unacceptable". Primary care trusts in low test areas said they were working to improve services for diabetes patients. According to National Institute for Clinical Excellence (NICE) guidelines, primary care trusts (PCTs) should ensure all diabetic patients are given a series of nine different health checks every 12 months. These include blood pressure, blood glucose levels, cholesterol, and examinations of the eyes and feet. Full Text

Drink coffee to cut diabetes risk

Want to stave off diabetes? Drink four cups of coffee a day, recommends a new study. Previous researches suggested that drinking coffee cuts diabetes risk but there were conflicting results on whether it protects or promotes chronic diseases such as cancer. Now, a team in Europe claims to have found that moderate consumption of coffee everyday may lower a person's chances of developing type 2 diabetes compared with those drinking it occasionally or not at all. In fact, drinking coffee can cut the risk of developing diabetes by nearly 30 per cent, says the study which has also revealed that the drink does not increase the risk of heart disease or cancer, the Daily Mail reported. Self-monitoring achieves significant benefit for newly diagnosed diabetics

A large scale review of self-monitoring of blood glucose (SMBG) reveals that newly diagnosed patients with type 2 diabetes, who are not on insulin, are achieving significantly better HbA1c values if they have access to blood glucose testing supplies. The less positive news is that the study showed HbA1c improvement to not continue bringing significant benefits for periods over a year, and this is the message that has been conveyed to health professionals through the media. Fitness experts say all that is needed is some space, a mat and a few favourite things. "Most important is that you have at least a five-foot by five-foot (1.5 by 1.5-meter) space, so that you're not worried about doing a movement full range, and a mat, so you've got a soft surface for work on the floor," said Lisa Wheeler, program director of DailyBurn.com, an online workout site. "Then just add what you like to do," she said. "If you don't like dumbbells, don't buy them." Wheeler's personal preference runs to resistance tubes. "I think they are a great piece of equipment because you can use them for several different exercises and you can travel with them," she said. For people who would rather not buy any equipment, Wheeler suggests using what is at hand. "You can use things you find around the house," she said.
