Question: When billing Medicare for diabetic shoes is the initial visit for evaluation and sizing considered the date of service or (if ordered) the day the shoes are delivered to the patient?

Question: I am part of the Medicare Therapeutic Footwear Program for diabetics. The processing company I'm currently using is changing how they do business making it very difficult to stay with them. Therefore, I'm looking for a software program I can use in my store that:
- Can access Medicare to verify eligibility of my diabetic customer.
- Can submit the sale to Medicare for payment.
- Can access secondary insurance companies (e.g. Blue Cross/Blue Shield) to verify eligibility.
- Can submit the sale to the secondary insurance company for payment.
Does anyone know of any software package that does all of the above or can name a company that will do all of the above and only collect their fee when I receive complete payment from Medicare and the secondary insurance companies. Thank you.

Question: This question is for those independent footwear retailers, who have undergone an onsite survey and also are looking to maintain accreditation with the ABCOP. I would like to hear from those that have footwear retail as their primary source of business. While it's imperative we have guidelines we must maintain and follow, I think there are some expectations or rules that can be unreasonable for an independent retailer. At times it seems like even with all our ducks in a row the inspector ends up looking for the proverbial "Red Herring."
Example: Private Area solely dedicated for evaluations.
Disposal container for sharp objects.
Fire drills and escape plans.
State compensation laws postings.
Is it possible that their expectations or the rules in place can be a little unreasonable? I'm primarily asking this of independent retailers without the resources of a full scale lab or school on their site. I would like to hear if some of these rules have an adverse affect or demand on your business and time.

Response: ABC's Response: Every patient care facility, regardless of size, must have policies and procedures in place which address areas such as emergency preparedness, compliance with state laws and patient's privacy. ABC's mission is to establish and promote the highest standards of organizational and clinical performance which is necessary for providing quality patient care. Compliance with ABC and Medicare standards ensures that a facility is demonstrating an exceptional level of patient care.
Mr. Altoro's Concern: Private Area solely dedicated for evaluations.
ABC's Comment: Private areas are necessary to preserve patient privacy and dignity while assessing a medical condition. It can be an area divided from the waiting room with a curtain or other movable divider, where conversations will not be overheard. Private areas are not only addressed in ABC's accreditation standards, they are also a requirement of Medicare (see Medicare's Quality Standards).

ABC's Comment: Many Pedorthic patients have also been diagnosed with diabetes. In recognition of this, ABC standards require that facilities have a way to dispose of hazardous materials such as items that may have come in contact with body fluids from open sores/wounds. This can be as simple as red bags that are routinely dropped off at appropriate centers for disposal.

ABC's Comment: Fire, disaster and emergency preparedness plans are essential to any business regardless of their size. Medicare's Quality Standards are very clear - you must have a contingency plan that allows the facility to respond to any emergency or disaster. Medicare expects that you will be able to provide beneficiary services regardless of the situation. ABC verifies the compliance of this requirement through documented drills which should be performed on an annual basis. Documented drills show that your facility is prepared and that you have identified areas that need to be addressed. For example, your fire drill would document where the designated meeting area is located, who does the head count, and who did the office sweep to make sure no one was left behind. Drills show that you have the ability to provide patient care regardless of what happens.

PREVIOUS QUESTION WITHOUT RESPONSES Question: Pedorthists have always seen a lack of compliance in the Diabetic population. The recent CDC article states that Diabetes amputations are falling dramatically. The article states that foot and leg amputations fell after 1996, and mentions that it was also the decade when Medicare began paying for blood sugar monitoring and for protective shoes and other medical devices for elderly diabetics. What are you doing as an important team member in teaching your patients compliance and reducing the amount of amputations?

| Importance of Foot care for Diabetics: Regular checks are Vital for healthy living
Our feet are our foundation, and it's important to take good care of them. But good foot care is even more crucial for a diabetic. Diabetes isn't just a blood sugar problem; the disease affects the entire body, and that includes nerves and circulation to the feet. However, foot problems often go unnoticed because diabetics tend to lose feeling in the feet over time. Dr. Michael Nirenberg, a podiatrist at Friendly Foot Care in Crown Point, sees this problem rather frequently and strongly recommends that diabetics see a podiatrist once a year. A podiatrist will examine the foot and check the strength of the pulse in the foot. A pulse that is too strong or not strong enough can be an early indicator of circulation problems. Paula Deen, the queen of Southern cuisine, created a hoopla last month when she revealed that she has type 2 diabetes and has known about it for three years. Some people were troubled that even after she was diagnosed, she continued to cook high-fat, high-sugar, high-calorie recipes on her TV show - foods that people with the disease are advised to consume infrequently. Deen said she eats such foods only in moderation. Meanwhile, Alec Baldwin told Entertainment Tonight he lost 30 pounds since being diagnosed with prediabetes in May. He says he gave up sugar, which was a real "killer" for him. So what's the best diet for people with diabetes? There is no one diet, whether it's a Mediterranean, low-carb or low-fat diet, that is consistently better at helping people manage diabetes, says Stephanie Dunbar, director of nutrition and medical affairs for the American Diabetes Association.
