NEW QUESTIONS AND COMMENTS
Question: I fitted a patient recently with therapeutic footwear and DF inserts which I believe are a proper fit. The patient came back for a follow up and stated that shoes are too big and cause heel slippage on the right foot (which is also her 1/2 size shorter foot). I gave the patient a heel slippage explanation, and also explained about correct diabetic shoe fit. This explanation hit deaf ears, and the patient insisted that the shoes are just too big. The patient also claimed redness and irritation at the heel. Evaluation did not show either. I added a tongue pad to the right shoe and also instructed patient to properly close the shoe (strap was worn very loose) and patient stated that it made it fit better.
Patient then had a follow up with a Podiatrist and the patient complained to the Podiatrist. Podiatrist held the sole of the shoe to patient's foot (did not actually fit check with the shoe on!) and stated that the shoe is almost 12 cm too long and that this is the poorest fit he had ever seen. The Podiatrist also dictated this in his notes and he asked for a shorter shoe. A reevaluation showed, again, proper shoe fit. I explained to the patient that I would not dispense a shorter shoe and that she would have to see one of our other CPed's. The patient complained to the physicians and I was told to dispense a shorter shoe. I declined and was told this may have consequences. Has any ever dealt with a situation like this?

Question: With Sanford Noblot indelible pencils having been discontinued, has anyone found a good alternative? I have found that the pencils that Tattoo artists use are a close second, but are not quite dark enough for my needs. I still have a couple dozen of the Noblots left, but would like to find an equal alternative ASAP.
- Rick E. Sevier, BOCO C. Ped. Question: I would like to repost this question, in hopes someone could share some insight. We've been using Riecken's PQ for years, but with the changes to the original formula that have been made, our lab techs are finding it to be increasingly more difficult to use and predict the firmess/cushion of the finished product. I've tried QuikSil and Viscolas in the past, but don't like them because you can't glue to them. I also tried Acton, but it too is difficult to keep in place. Does anyone have any other suggestions for alternative viscoelastic polymers to be used in foot orthoses? Thanks. - Erick J. Janisse, C.O., C.Ped. PREVIOUS QUESTION WITH RESPONSE Question: I am a 30 year old runner who started running in my early 20's. Either due to genetics or improper footwear for the past 10 years, I have noticed that my left bunion gets worse every year. As the bunion is getting more pronounced, I have noticed that I will have dull pain at the end of the day or at the end of a run in my second toe. Is there something I should be doing to help relieve the bunion/second toe pain? Thank you.

Response: As the bunion becomes more pronounced, it is possible that the great toe is also turning back toward the second toe, which creates the effect of shortening the length of the great toe and producing a Morton's toe situation. For a runner, this situation means that the toe-off is transferred more toward the second toe, putting an extraordinary load on that toe. If this is the case, there are orthotics that employ Morton's toe corrective devices, as well as a flat metal plate that can be used underneath the footbed to correct the toe-off motion and push it more towards the great toe where it should be. While these measures will help with running, they do not fully address the issue of the bunion.

Shoe Carnival Cuts Q4 EPS on Weak Boot Sales Shoe Carnival said poor boot sales, largely due to warm weather so far this winter season, led to a fourth-quarter EPS shortfall. The family shoe chain expects EPS in the fourth quarter to be in the range of 20 cents to 23 cents a share, down from an earlier projection of 33 cents to 36 cents. Comparable store sales are expected to decline in the range of 3.0 to 3.5 percent.In releasing third-quarter results on Nov. 17, Shoe Carnival expected comps to grow 4.6 percent. Total sales are now expected to be in the range of $181 to $182 million versus a previous target of sales in the range of $186 to $190 million. | Few With Diabetes Risk See Need for Lifestyle Counseling
Many patients who are at risk for developing type 2 diabetes do not perceive the need for lifestyle counseling, and even among those who see the need for counseling, less than half actually attend lifestyle interventions, according to a study published online Dec. 21 in Diabetes Care. Sanna M. Salmela, of the University of Jyväskylä in Finland, and colleagues analyzed baseline and intervention data from 10,149 participants in the Finnish National Diabetes Prevention Project. Three cups of black tea each day could help reduce risk of diabetes A new study has claimed that people who drink at least three cups of black tea each day could be lowering their risk of developing conditions such as diabetes and heart attacks.It already believed that regularly drinking tea helps to prevent the blood from clotting and manages blood pressure. However, this research, based on analysis of 40 research papers and published in the UK Nutrition Bulletin, found that three cups of tea per day could reduce the risk of heart attack by up to 60 per cent and substantially lower the chances of people getting type 2 diabetes. Many diabetics could be misdiagnosed, says report
A new study from the Royal College of General Practitioners and NHS Diabetes has claimed that many people who have been told they have type 2 diabetes could actually be healthy.The research, based on information from 45,000 patients at five doctor's practices in Surrey, showed that two per cent of the patients diagnosed with type 2 diabetes did not suffer from the metabolic condition at all. If these findings were extrapolated across the whole of the UK, it could account for about 50,000 cases of misdiagnosed diabetes. Full Text

This class completes required education prerequisite to sit for national certification exam. It does not satisfy the requirement of 1000 documented hours of field experience.
